Where Cables which are installed in hazardous areas introduce the risk of fire or explosion in the event of an electrical fault in such areas, special precautions against such rises shall be taken to the satisfaction of the administration. PSCO observed multiple fractures on electrical conduit on entire length on cargo block.

The arrangements for ready availability of water supply shall be to the satisfaction of the administration. PSCO observed fracture at the flex coupling on fire main.

Each incinerator installed on a ship shall meet the requirements contained in appendix IV to this annex. PSCO observed pieces of the incinerator’s refractory door chipped or missing. 50abc MARPOL VI/16.6.1

Fire protection systems and fire fighting systems and appliances shall be maintained ready for use. PSCO observed the fire main drain valve ceased closed on the main deck, starboard side near the number one cargo hold. 17ABC 74 SOLAS (09 Cons.) II-2/1.1

Electrical installations shall be such that the safety of crew and ship from electrical hazards will be ensured. PSCO observed exposed electrical wiring below the emergency stop junction box inside the fire control station. 17ABC 74 SOLAS (20) II-1/40.1.3

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed 07 deformed fire extinguisher hoses in the engine room. 17ABC 74 SOLAS (09 Cons.) II-2/14.2.1.2

Bulk carriers shall be subject to an enhanced programme of inspections in accordance with the International Code on the Enhanced Programme of (2011 ESP Code). Checking of the satisfactory operation of all mechanically operated hatch covers should be made, including proper fit and efficiency of sealing in closed condition. PSCO observed that the rubber sealing gaskets of the number one cargo hold hatch were cracked and worn in several places. 17ABC 74SOLAS (20 Cons.)XI-1/2/ESP Code 2011 3.3.2.2

Every ship of 100 gross tonnage and above, and every ship which is certified to carry 15 or more persons, and fixed or floating platforms shall carry a garbage management plan which the crew shall follow. The PSCO observed garbage stored in a compartment marked as garbage store on the poop deck. The garbage was not segregated or color-coded as required by their Garbage Management Plan instead, left on the deck and stuffed in a room. 16ABC MARPOL (11 Cons.) Annex V / Reg. 10.2

The ring, lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. The PSCO observed the deck line to be painted in the same color as the ship, contrary to the required contrasting color specifications. 10C ICLL 66 Annex I / Reg. 8

All the steering gear components shall be of sound and reliable construction to the satisfaction of the Administration. The Steering Gear reservoir tank was leaking at one of the pipe connections. 17ABC 74 SOLAS (2020) II-1/29.2.1

To prevent the ignition of combustible liquids and flammable liquids, means shall be provided to control leaks of flammable liquids. PSCO observed substantial fuel oil in the Fuel Oil Purifier space. Fuel oil was accumulated and pooled below the deckplate adjacent to the HFO Service Tank. The lagging along the wall of the HFO service tank was saturated in fuel oil and dripping through the metal sheathing cover panels with multiple seams along the sheathing that was bleeding fuel oil through. The connection to the HFO Service Tank was leaking. 17ABC 74 SOLAS (09 Cons.) II-2/4.1.1.1

PSCO observed fuel oil throughout various areas of the engine room to include: A fuel oil leak on the Main Engine #6, oil soaked lagging on the #6 Fuel Oil return line on Main Engine, a drain pipe leaking at valve on the Main Engine #1, a leak on the Main Engine Turbo Charger drain pipe, pooled fuel oil under all three generators, a fuel transfer pump in the bilge area of the engine room with pooled fuel oil under the strainers inside secondary containment pan, fuel oil soaked lagging around the strainers, and fuel oil soaked lagging on piping. 17ABC 74 SOLAS (09 Cons.) II-2/4.1.1.1

Drills shall, as far as practicable, be conducted as if there were an actual emergency. PSCO observed 02 firefighting drills. During both firefighting drills the crew demonstrated a lack of competence, inadequate training, preparation, proficiency, and communication. 17ABC 74 SOLAS (20) III/19.3.1

The condition of the ship and its equipment shall be maintained to conform with present regulations and to ensure the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed the #1 Cargo Hold's hatch cover hydraulic cylinder on the forward port side had a broken seal and observed active leakage. Additionally, there was a hydraulic oil leakage observed from the #1 Cargo Hold hatch cover hydraulic cylinder (aft starboard side) and #2 Cargo Hold hatch cover hydraulic cylinder (aft port side). Changed to 16ABC 74 SOLAS (2020) I/11(a)

The company and the ship shall comply with the requirements of the International Safety Management code. The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. PSCO observed various technical and/or operational deficiencies regarding the maintenance of the vessel and proficiency of the crew. External audit required to address items numbered 02-16 below. 17ABC 74 SOLAS (20) IX/3.1 ISM Code 10.1

The provision of safe means of access to and throughout the ship, and the satisfactory physical conditions of ladders, decks, bulwarks, fencing, handrails. etc. PSCO observed 01 grating on the starboard bow and support structure thereof that was deteriorated, rusted through, and wasted. The quick closing watertight door leading to the steering gear flat ladderwell was also split in half after an attempt to open it. 50ABC ILO 147 prevention of occupational accidents/structural features of the ship/ paragraph 1 & PWSA 46 USC 70002 & 33 USC 1223(b)

Every ship of 400 gross tonnage and above, shall carry a Garbage Management Plan which the crew shall follow. PSCO observed the C/O was incinerating plastics (Category A) when Garbage [name withheld] states “Company does not allow vessel to incinerate plastics even if vessel is certified to,” in Chapter 11. MARPOK V/10.2

In the case of periodically unattended machinery spaces, the fire extinguishing system shall have both automatic and manual release capabilities. PSCO observed the water mist system installed was incapable of being placed in automatic mode yet both Minimum Safe Manning Certificate and Class Certificate state vessel can operate as a “UMS”. Request clarification from certificate issuing Authority. 74SOLAS CONS. 11-2/10.5.6.2

The Company should establish procedures to ensure that new personnel and personnel transferred to new assignments related to the safety and protection of the environment are given proper familiarization with their duties, Vessel went under new management and was issued a new Garbage management Plan on 15SEP23, but C/O did not know incineration of plastics was not allowed under new plan. Request internal audit is conducted. ISM 6.3

Each discharge into the sea or to a reception facility, or a completed incineratrion, shall be promptly recorded in the garbage record book and signed for on the date of the discharge or incineration by the officer in charge. PSCO observed that no entries had been mae in garbage record book part II, despite washwater and cargo residue being discharged.
