The machinery, boilers, and other pressure vessels, associated piping systems fittings shall be of a design and construction adequate for theservice for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. 74 SOLAS (04) II-1/26.1 PSCO observed the packing gland on the ballast water pump leaking, packing gland on the HFO pump leaking, generator #1 jacket water leaking, and a leak on the main engine fuel return line.
Condition: Improper/Lack of Maintenance
Action required: 15 - Rectify deficiencies by next port

Resolution: Packing glands replaced, leaks repaired. All certified by Bureau Veritas.

Electrical installations shall be such that: the safety of the passengers, crew, and ship from electrical hazards will be ensured. 74 SOLAS (20) II-1/40.1.3 PSCO observed exposed wires on the light fixture above the switchboard in the emergency generator room and the circuit breaker in ECR for BWTS control cabinet was a higher rating than what was indicated on the panel.

Resolution: The split wiring at light fixture for the emergency generator was replaced for a cable wiring. The labelling for the circuit breaker showing 15A was replaced to show 30A as indicated on the breaker. Examined satisfactorily by BV.

Master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the following ballast water management system (BWMS)that has been approved by theCoast Guard...the master, owner, operator, agent, or person in chargeof vessles employing a Coast Guard approved BWMS must meet the applicable ballast water discharge standard (BWDS). 33CFR 151.2025 (a)(1) PSCO observed BWTS exhibiting "UV intensity USCG low, system flow maintained at 50%, Process not compliant." which indicates system may not be operating as intended.

Resolution: Sensor was replaced as a preventative measure. The captain confirmed that they have reached out to the system manufacturer to get their response on the matter of compliance.

Unless otherwise expressly provided for, the master, owner, operator, agent or person in charge of vessels employing a coast guard approved BWMS must meet the applicable ballast water discharge standard, at all times of discharge into the waters of the United States. Vessel discharged approx. 20396m3 of untreated ballast water into local waters due to inoperable BWTS. Vessel was unable to treat ballast water due to bypassing U/V filters. 33CFR151.2025(a)(1)
