Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500L and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. Remote Fuel shutoff to Emergency Diesel Generator was not operational from outside the space, preventing the securing of a fuel source in the event of a fire.

Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gas tight and self-closing. Doors 011 and 002 leading to stairwells and escape trunks in the engine room boundary were found out of alignment preventing full closure and increasing risk of fire spread outside of the category A machinery space.

Deficiencies identified by the PSCO and Flag State Control issued 18-Feb-2025 indicate a serious failure, or lack of effectiveness, of the implementation of the ISM Code. An external safety management audit has to be carried out before the ship may be released from detention.

A fixed fire detection and fire alarm system shall be installed in: machinery spaces where: the installation of automatic and remote control systems and equipment has been approved in lieu of continuous manning of the space. The Smoke detector in the Purifier space was found out not operational. Vessel operates periodically unattended machinery space.
