Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed 32 out of 41 immersion suits unserviceable due to glue failure and missing retroreflective tape.

For tankers of 20,000 tonne deadweight and upwards constructed on or after 1 July 2002 but before 1 january 2016, the protection of the cargo tanks shall be achieved by a fixed inert gas system in accordance with the requirements of the Fire Safety Systems Code. (74 SOLAS 20 II-2/4.5.5.1.1) The system shall be designed to ensure that if the oxygen content exceeds 5% by volume, the inert gas shall be automatically vented to atmosphere.PSCO observed the inert gas main line valve not opening/closing automatically.
