THE OBJECTIVE EVIDENCE OF THE DEFICIENCIES BELOW ALONG WITH THE LACK OF CREW REPORTING OF NON-CONFORMITIES AND TAKING APPROPRIATE CORRECTIVE ACTIONS BRINGS INTO QUESTION THE VALIDITY OF THE SAFETY MANAGEMENT SYSTEM AS REQUIRED IN THE ISM CODE. AN AUDIT IS REQUIRED TO BE SUBMITTED TO THE COAST GUARD PRIOR TO DEPARTURE.

THE WATER CURTAIN ON THE STARBOARD SIDE HAD TWO BROKEN BRACKETS THAT PREVENT THE WATER CURTAIN FROM ADEQUATELY PROTECTING THE HULL.
