Vessel experienced propulsion irregularities while transiting in the Sector New Orleans Captain of the Port Zone on 21 AUG 2022. PSCO observed the vessel failed to immediately report the hazardous condition to the nearest Coast Guard office until 24 August 2022.

The company and the ship shall comply with the requirements of the International Safey Management Code. For the purpose of this regualation, the requirements of the Code shall be treated as mandatory. The safety management system should include procedures ensuring that non-conformities, accidents, and hazardous situations are reported to the company, investigated and analyzed with the objective of improving safety and pollution prevention. PSCO observed ship failed to submit non-conformity reports without delay in accordance with the company's safety management system. Documentation provided to PSCO showed non-conformance form submitted to company on 25 AUG 2022 for a hazardous situation that occured on 21 AUG 2022.Conduct internal audit within 30 days.
