As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. (b) Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Vessel had outdated pubs.

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Life ring was missing retro tape.

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Fuel shut offs were not labeled.

REPAIRS OR ALTERATIONS TO HULL, MACHINERY, OR EQUIPMENT THAT AFFECTS SAFETY OF THE VESSEL, MUST NOT BE MADE WITH THE APPROVAL OF COGNIZANT OFFICER IN CHARGE OF MARINE INSPECTION (OCMI) THE VESSEL IS PREPARING TO FIBERGLASS THE BOTTOM. DURING SURVEY OF HULL AND DIALOG WITH OWNER ALOT OF WOOD REPLACEMENT WAS DISCOVERED TO BE NECESSARY BEFORE GLASSING CAN BE STARTED. SUBMIT REPAIR PROPOSAL TO REPLACE OR REPAIR AS ORIGIONAL CONSTRUCTION ANY BOTTOM PLANKS, FRAMING, FLOOR BEAMS, ENGINE MOUNTING, DECK BEAMS, DECKING, BULL WORKS, OVERHEAD CANOPY AND FASTENERS. PROVIDE LINE DRAWINGS FOR ANY ELECTRICAL, MECHANICAL OR VITAL SYSTEMS AS APPLICABLE. 46 CFR 176.600

Shutoff valves shall be fitted in the fuel supply lines, one at the tank connection end of the fuel line. Vessel's fuel shutoff valve at PORT fuel tank did not close properly at time of inspection. 46 CFR 182.115(a) "Old T" 182.20-40(b)(3)

Portable fire extinguishers shall be installed and located to the satisfaction of the [name withheld] portable extinguishers mounted near PORT and STBD sides of operating station require suitable marine brackets suitable for marine use. 46 CFR 181.115(a) "Old T" 181.30-10

Vessels constructed of wood must provide bilge high level visual and audible alarms in all watertight compartments. Vessel's high water visual alarm inoperable at time of inspection. 46 CFR 182.115(d) 46 CFR 182.530(b)

A vessel must carry either a first-aid kit with contents stowed in a suitable, watertight container that is marked “First-Aid Kit”. Vessel's first-aid kit contained expired equipment. 46 CFR 184.710

Each buoyant apparatus on the vessel on March 11, 1996, may be used to meet the requirements of a survival craft as long as the survival craft is continued in use on the vessel, and is in good and serviceable condition. Vessel buoyant apparatus approval plate illegible at time of inspection. Buoyant apparatus must be replaced with serviceable lifefloat by cold water months (Nov 1). 46 CFR 180.15(d)

At least one ring life buoy must be fitted with a lifeline. The lifeline was found to be missing.

Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length, but not more than 6 feet, securing the waterlight around the body of the ring life buoy. Improper measurements for waterlight lanyard observed.

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Steering gear to include rudder post hardware and gasket material loose/wasted. Connection from steering ram to attachment on rudder post highly worn - repair or replace. Identify and secure source of water intrusion from rudder post.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. First aid kit was missing items as required by 46 CFR 160.054-4.

Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 0.5 inches high: “DISTRESS SIGNALS”. Vessel Operator did not have a container for flares.

46 CFR 177.1020 Each window, port hole, and its means of attachment to the hull or deck house, must be capable of withstanding the maximum load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. Port side operating station window was found to be cracked.

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. Crew training to include abandon ship, fire drills and man overboard not properly logged and/or missing.
