The Company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The Safety Management System should provide for specific measures aimed at promoting the reliability of such equipment or systems. These measures should include the regular testing of equipments. PSCO observed during the Damage Survey that the vessel does not have establish maintenance procedures for the Alpha lubricator cylinder oil pressure pump transmitter sensor that caused the main engine propulsion irregularity. The company shall provide a corrective action plan to establish procedures with the concurrence of the vessel administration or the certificate issuing authority.
