The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCOs identified excessive oil leaks, oil accumulation, and oil soaked lagging throughout the main engineering space, including the purifier flat, waste oil tank, generators and main engine due to insufficient maintenance of ship's systems. After any survey of the ship under regulations 7, 8 9 or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. PSCOs identified a flexible hose affixed to a freshwater line in order to fill the cascade tank due to a salinity imbalance in the boiler feed system. Crew failed to correct the root cause of the imbalance. 74 SOLAS (14) I/11(b), 17ac

The ignitability of combustible materials shall be restricted. The vessel had a fire in the [name withheld] cabin due to the ignition of combustible materials on or about 10AUG2020. The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCOs identified failed, leaking temporary repairs on sea water cooling lines for the generators and main engine air cooler. 74SOLAS(14)II-1/26.1, 17ac

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCOs identified leaking at the seawater intake pump for the freshwater generator. Additionally, there is an active steam leak at the freshwater heating pipe for the freshwater generator. Crew has been only using water directly taken from the freshwater generator for cooking purposes, instead of installed faucets. Crew was unable explain the reason for drawing cooking water directly from the freshwater generator. 74 SOLAS (14) II-1/26.1, 17ac

All electric cables and wiring external to equipment shall be at least of a flame-retardant type and shall be so installed as not to impair their original flame-retarding properties. PSCOs identified broken solenoid housing exposing electrical wires on the incinerator. 74SOLAS (14)II-1/45.5.2, 17ac

Provision shall be made to facilitate cleaning, inspection and maintenance of main propulsion and auxiliary machinery including boilers and pressure vessels. PSCOs identified missing isolation valve handles on the fuel supply lines to the No. 4 and No. 5 fuel pumps for the main engine.74 SOLAS (14)II-1/26.7, 50ac
