The vessel experienced an equipment failure while transiting the navigable waters of the united states which adversely affected its maneuverability and posed a hazard to vessels and infrastructure along the Houston ship channel. Submit for review a classification society report to [email withheld] attesting to the cause of the equipment failure that authorized temporary or permanent repairs have been completed and that the vessel's full and normal maneuverability has been restored. 74SOLAS(14)II-1/26.1

PSCO observed a steady fuel leak dripping from supply line to the burner of the ships boiler, oil soaked lagging was observed in two locations (1) bunker pipeline in the overhead of the bottom deck, port side of the engine room (2) a pipe below the deck plates, next to the main engine turning gear. 74SOLAS(14)II-2/4.1.1

Fire hoses shall be of non-perishable material and shall be sufficient in length to project a jet of water to any of the spaces in which they may be required to be used. Each hose shall be provided with a nozzle and the necessary couplings. The bridge wing fire hose nozzle could not change to the spray function and PSCO observed two fire hoses in the bottom deck of the engine room using hose clamps and tape to connect the hose liner to the couplings. 74SOLAS(14)II-2/10.2.3.1.1
