A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: 1. light to indicate discharge; 2. audible alarm that sounds upon discharge; 3. means to reset devices used to automatically shut down engines as required. The audible and visual alarms were found inoperable, and the control was found hanging lose and not secured to the operating stations. 46 CFR 181.420 (b).

Individual power pumps used for separate spaces are to be controlled from a central control point and must have a light or other visual means at the control point to indicate operation. All 03 bilge pump visual light at the operating stations were found inoperable. 46 CF 182.520 (a).

A portable hand bilge pump must be: 1. Capable of pumping water from all watertight compartments; 2. provided with suitable suction hose capable of reaching the bilge of each watertight compartment and discharging overboard. Portable hand bilge pump was found bend and inoperable. 46 CFR 182.520 (b).

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces. Port and stbd engine compartment fire ventilation dampers were found with gaps and not to be air/gas tight. 46 CFR 177.405 (e).

A submersible electric bilge pump may be used as a power bilge pump. The engine room submersible bilge pump was found inoperable upon testing. 46 CFR 182.520 (e).

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces. Cable penetrations into the engine room from the forward void were found not sealed and not air/gas tight. 46 CFR 177.405 (e).

Exhaust gas must not leak from the piping or any connections. Exhaust gas leak was identified in the portside engine. 46 CFR 182.430 (b).

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. Small lifefloat was found not having a painter line attached to a weaklink, and without a weaklink permanently attached to the vessel. 46 CFR 180.130 (a).

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. Large lifefloat painter line was not permanently secured to the weaklink. 46 CFR 180.130 (a).

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Small lifefloat was found missing a painter line, water light, and both paddles were found wasted due to UV exposure. 46 CFR 180.175 (d)

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Large lifefloat was found with an inoperable waterlight and both paddles wasted due to UV exposure. 46 CFR 180.175(d).

Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length, but not more than 6 feet, securing the waterlight around the body of the ring life buoy. Waterlight was secured to the end of the lifeline, instead of having its own line around the lifering. 46 CFR 180.70 (d)(2)

Each ring life buoy on a vessel must:(1) Be approved in accordance with § 160.050. The lifering grabline was found wasted due to UV exposure. 46 CFR 180.70 (b)

A seat must be constructed to minimize the possibility of injury and avoid trapping occupants. Upper deck passenger benches were found missing screws in the portside and not even connected to the canopy aluminum superstructure in the starboard side. 46 CFR 177.820 (b)

At each initial and subsequent inspection for certification, the owner or operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including Inspection and testing of all smoke and fire detection systems, including sensors and alarms. A smoke detector is missing in the steering compartment; and conduct testing of the smoke detection system, including the detection panel located in the forward compartment. 46 CFR 176.810

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Conduct drills in presence of an attending USCG Marine Inspector.46 CFR 185.520

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 1 inch numbers and letters at the entranceway to that deck. Upper deck was missing marking with max allowance of passengers.46 CFR 185.602 (g)

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Provide evidence of training; and provide the EAP Placard. 46 CFR 16.401

All-round light means a light showing an unbroken light over an arc of the horizon of 360 degrees. All around light was found inoperable, and missing dayshapes. 33 CFR 83.21 (e)

Electrical equipment on a vessel must be installed and maintained to: (a)provide services necessary for safety under normal and emergency conditions;(b)Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock;(c)Minimize accidental personnel contact with energized parts; and (d)Prevent electrical ignition of flammable vapors. The following were found not in compliance: 1. Ground wire not connected in the 2nd void. 2. 02 electrical junction boxes without cover in 2nd void. 3. 02 ground wires not connected under the operating station 4. dead-end wires found under the operating station. 46 CFR 183.200

An adult life jacket must be provided for each person carried on board a vessel. Vessel was missing 01 adult-sized life jacket. Vessel is restricted to not more than 51 persons until a lifejacket is provided.46 CFR 180.71 705 restricted to 51 persons.

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. It normally includes inspection and testing of the structure, machinery, equipment, and on a sailing vessel, rigging and sails. The owner or operator must conduct all tests as required by the OCMI, and make the vessel available for all specific inspections and drills required by subpart H of this part. In addition, the OCMI may require the vessel to get underway. Vessel failed to pass and finish a Certificate of Inspection exam. Vessel Owner/Operator shall request a new Certificate of Inspection exam once all 21 deficiencies have been corrected, and notify the OCMI that vessel all systems have been tested and found satisfactory prior requesting thee exam.46 CFR 176.404

FRP delamination was found in the stbdside hull next to the keel midship. Code 705 Prior dry dock credit, 16C.

Hatches for the collision bulkhead and steering compartment were found leaking water inside the compartment. Service these hatches to make them watertight upon testing by USCG Marine Inspector. Code 705 Prior dry dock credit, 16C.

Two structural deck support frames connecting with the transom panel were found fractured and delaminated. Properly repair and secure with strong bond.

A smoke detector was found hanging lose in the steering compartment. Secure and retest the smoke detector. Code 705 Prior dry dock credit, 16C.

The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. Shutoff valve was inoperable. Repair. 46 CFR 182.455(b)(4)

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Engine bilge alarm light is not operational. Repair. 46 CFR 182.530(a)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Vessel's life float is worn. Replace life float. 46 CFR 176.808(b) 705c - 4months

46 CFR 181.420(a)(e)A pre-engineered fixed gas fire extinguishing system must:Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. The Pre-engineered system was found inoperable when tested. Complete repairs and test to the satisfaction of the attending inspector.

46 CFR 185.728 (b) The EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner. Marine inspector witnessed the battery was expired. Vessel is restricted to operations within 3NM of shore until service is complete to the satisfaction of the marine inspector.

46 CFR 185.740 (b) Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Marine inspector witnessed the hydro static release was expired. Vessel is restricted to operations within 3NM of shore until replacement is completed to the satisfaction of the marine inspector.

46 CFR 182.100 All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Marine inspector witnessed temporary repairs on the starboard engine exhaust pipe. Repair or replace to original to the satisfaction of the attending marine inspector.

46 CFR 184.420 (a)(1) As appropriate for the intended voyage, a vessel must carry adequate and up-to-date Charts of large enough scale to make safe navigation possible. Vessel did not have paper or electronic charts on board. Provide vessel with charts to the satisfaction of the attending marine inspector.

46 CFR 181.610 A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Ammonia Capsule were not on board. Replenish fist aid kit with missing contents to the satisfaction of the attending marine inspector.

46 CFR 181.610 A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Only two fire buckets were on board the vessel at the time of the inspection. Provide a third fire bucket to be kept on board to the satisfaction of the attending marine inspector.

Resecure/renew fire insulation in overhead of engine compartment at aft centerline and stbd fwd above stbd fuel tank. 7 Days provided to correct deficiency to allow vessel to splash. Extended to 30 days to correct.

Renew port engine crankcase ventilation system filter housing lid. Current lid is cracked and disintegrating.

Renew deteriorated red control cable run for aft control steering station. 30 days provided to rectify deficiency.

Spaces containing diesel machinery must have provision for closing all supply duct cowls or scoops that are protected by a fixed gas extinguishing system. The engine compartment starboard ventilation damper did not properly close.

Each item of survival craft equipment must be of good quality and efficient for the purpose it is intended to serve. The starboard side life float light did not illuminate.

A vessel must comply with the applicable requirements for any radio installations and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel's Safety Radiotelephony Certificate was expired.

Gauges to indicate engine revolutions per minute, jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. Starboard engine tachometer and analog battery voltage meter provided inaccurate readings.

A visual and audible alarm must be provided at the operating station to indicate a high water level in normally unmanned spaces. The steering compartment bilge alarm light is inoperable.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards. Dead end wires under the operating station and on the starboard side of the void forward of the engine compartment require removal.

For each marine casualty required to be reported by 185.202, the owner, agent, master, or person in charge of the vessel shall determine whether there is any evidence of alcohol or drug use by individuals directly involved in the casualty. The vessel's alcohol test swabs were expired.

Propulsion machinery must be suitable in type and design. Found the following: a. the U-bolts of the forward bridge throttle controls is wasted; b. the aft throttle control station the U-Bolts for the cables wasted away and was repaired by a non-approved method. 46 CFR 182.200(a).

Gauges to indicate RPM must be provided for all propulsion engines installed in a vessel. The portside bridge RPM gauge reads 600 RPMs with the engine stopped. 46 CFR 182.410 (b).

The discharge ends of fuel tank vents must terminate on the hull exterior, as high above the water as practicable. Found both port & starboard fuel tank vents terminates inside the engine room. Unable to determine if the vent lines are fitted with flame screens in accordance with 46 CFR 182.450 (c). 46 CFR 182.450 (d) (e).

Provisions must be made for closing all supply ducts for a space protected by a fixed gas extinguishing system. Both port & starboard engine room supply ducts do not completely close when the covers are activated. 46CFR182.465(b).

A hatch exposed to the weather must be watertight. Found the following hatches not being watertight: a. bow deck hatch is leaking excessively; b. steering compartment hatch was found leaking excessively and not attached to the hatch frame by a captive chain or other device; c. compartment forward of the engine room was found the hatch to be missing the rubber gasket seal and the securing devices are wasted and non-operational and not attached to a captive chain or other device. 46CFR179.360

All observed unsafe practices and other hazardous situations must be corrected. Series of holes were found at the height of the rub rail, portside at the forward void (anchor chain locker). Properly seal holes or install proper correct bolts. 46 CFR 176.830 (a)

The lights prescribed by these rules shall, if carried, also be exhibited from sunrise to sunset in restricted visibility. The following navigation lights must be: a. starboard side was found UV damaged and needs to be replaced; b. stern light must be mounted as far aft as possible and visible in accordance with the regulation; c. Anchor light was found with UV light damage and needs to be replaced. 33 CFR 83.20

Vessel is required to have on board a portable hand bilge pump in accordance with 46CFR 182.520 (a). Vessel portable bilge hand pump was found to be broken and not working. Provide a working portable bilge hand pump. 46CFR182.520(a).

Each window and its means of attachment to the hull or deck house, must be capable of withstanding the maximum load from wave and wind conditions. Main deck center window frame was found broken, with glass being supported by a screwdriver. Properly conduct repairs on center window. 46CFR177.1020.

Vessels certificated to operate on a limited coastwise route must have on board a FCC type 1 406Mhz EPIRB installed in a float-free position. The EPIRB hydrostatic release was found to be expired. Replace with a valid hydrostatic release. 46CFR 180.205 (3).

Each portable fire equipment (extinguishers) on a vessel must be of an approved type. The operating station fire extinguisher was found to have a broken support or bracket. Provide an approved bracket or support according with the fire extinguisher label.

An emergency instructions placard must be tailored to the vessel operations and existing emergency equipment. Vessel has posted the recommended emergency instructions format and must be updated. 46 CFR 185.512.

Each lifeline on a ring life buoy must be of a dark color if synthetic, or of a type certified to be resistant to UV deterioration. The lifeline was found to be light in color and not be UV resistant. 46 CFR 180.70 (c) (b).

A vessel must comply with the applicable procedures and record requirements of 33CFR151.57. Vessel was found not having a waste management plan on board. 46 CFR184.702/ 33CFR151.57.

On a vessel on which the number of passengers permitted in the upper deck is limited by stability criteria, as indicated by stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 1” numbers, at the entranceway to that deck. Provide allowed number of passengers permitted in the upper deck in accordance with current stability letters. 46CFR185.602(g).

All cable and wire must be installed with metal supports spaced not more than 24” apart, and in such a manner as to avoid chafing and other damage, and as high above the bilges. The following cables and wiring needs to be supported: a. portside flood light electric cable needs to be secured; b. Electric boat cable connected to the starboard side battery was found hanging and making contact with the hull; c. secure or remove dead-end wires found underneath the operating station controls. 46CFR183.340 (b)(4)

FWD bilge pump was not UL listed

Life floats were not marked with vessel name.

Life floats were not fitted with paddles.

Paddles were not marked with vessel name.

Life floats were not separated by spacers.

Waterlight was not lashed around ring life buoy with corrosion resistant clip.

First-aid kit did not have all required contents.

Child life jacket stowage was not properly marked.

MOB and Fire drills were not logged.

EPIRB tests were not being logged monthly.

Vessels all-round white anchor light was inoperable.

Portable lights had inoperable batteries.

The EPRIB on board had a battery expiration date of 2017 and the antenna was damaged. Vessel may not operate past (03) nm until EPRIB is replaced.

The COI and Stability Letter were not posted on the vessel.

Vessel must replace all plastic zip ties in the engine room with proper metal supports.

Each life float must be fitted with two paddles. The vessel’s life floats do not have paddles.

The vessel’s MMSI number was not entered into the radio.

08 lifejackets were found in poor condition. Limited to carrying no more than 44 persons on board.

Replace retroreflective tape on all life jackets

Install metal supports on cables under operating station.

Secure CPUs in engine compartment

Prove Proper Operation of Bildge High Water alarm System. 46CFR182.530 Prior to carrying passengers.

Provide complete first aid kit

Prove proper installation of inflatable life raft.

Replace the corroded fuel shut off cable on the port side.

Replace the corroded/broken wire on the port engine alternator.

Repair two cracked welds on the life raft release track and 4 welds on the canopy.

Fix broken grounding cable on the port side of the engine room.

Replace 16 gauge wires with the proper wiring for power and lightning.

Properly support the Port and STBD PC units in the Engine Room.

Make all strut bolts watertight

Properly secure all SCUBA bottle racks on the Port and STBD side.

Remove all deadended wires on the backside of breakers.

Prove proper operation of engine room and lazarette bilge high level alarm. Conduct Rounds every 30 minutes and/or prior to each departure.

Provide working waterlight. Vessel restricted to daytime operations and clear visibility.

Program MMSI Number in VHF-DSC Radio.

Relocate stern light to aft most rail ensuring clear visibility to vessel traffic.
