Each scuttle used as means of escape must be capable of being opened by one person, from either side, in both light and dark conditions. The method of opening the means of escape must be obvious, rapid, and of adequate strength.
The emergency escape scuttle from the galley into the dining room / main deck area has a carpet overlay that sits flush with the surrounding carpet which makes it difficult to open from the main deck. Provide a pull tab or similar arrangement that allows for rapid removal of the escape scuttle by a single person from the main deck.

Each ladder used as a means of escape must be mounted at least 7 inches from the nearest permanent object in back off the ladder. Rungs must be at least 16 inches in width and no more than 12 inches apart. Footholds, handholds, ladders, and similar means provided to aid escape must be suitable for use in emergency conditions and of rigid construction.
The emergency means of escape for the engine room requires stepping up onto a raised platform several feet off the deck in order to access the ladder, or stepping on a PVC pipe in order to step up onto the raised platform.
Update 3/27/23: extended deficiency due date to 6/14/23.

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 2 inches high: “EMERGENCY EXIT, KEEP CLEAR”.
Emergency escape scuttles for galley space and engine room are not marked on top of scuttle where the exit into the dining room space.
*On 3/18/22, this deficiency was extended to 4/15/22 and 'prior to carry passengers' requirement removed, based on temporary signage arrangements.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent.
Current arrangement for engine room ventilation covers/closures requires a crewmember to climb along the exterior side of the vessel, flip up the ventilation covers, and secure them with a screwdriver and screw.

All cable and wire must be installed with metals supports spaced not more than 24 inches apart…the use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support.
Several wiring runs found not properly supported, to include transducer wiring in tank compartment and camera wiring in galley space.
