The purpose of this regulation is to prevent the ignition of combustible materials of flammable liquids. During purifier room exam, PSCO identified large amounts of fuel oil accumulation and soaked lagging. 74 SOLAS (09) II-2/4.1

PSCO observed multiple leaks from flanges on both fresh water cooling systems and oil leaks from lube oil cooling system. 74 SOLAS (20) II-1/26.1

The arrangement for the storage, distribution and utilization of oil used in pressure lubrication system systems shall be such to ensure the safety of the ship and persons onboard. PSCO observed temporary repairs and leaking of lubricating oil from the reservoir tank for the fore shaft seal. There was also excessive accumulation of lubricating oil below the shaft seal. 74 SOLAS (09) II-2/4.2.3.1

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. PSCO discovered numerous items that holistically demonstrated a general lack of maintenance on board including, a ruptured fire hose, an unserviceable firefighting suit, an inoperable fire door, electrical hazards and a leaking hydraulic ram on a cargo hatch. The access between the machinery space and the space containing the emergency fire pump may be by means of an airlock with the door of the machinery space being of an A-60 class standard, and the other door being at least steel, both reasonably gastight, self-closing, and without any hold-back arrangements. Class A fire boundary door from the engine room to the vessel's steering compartment would not fully self close. 74 SOLAS (14) II-2/10.2.2.3.2.2 17C

Fire hoses shall be of non-perishable material approved by the administration and shall be kept ready for use in conspicuous positions near the water service hydrant or connections. While performing a test of the ships fire main, PSCO observed leaking from multiple locations along the length of the hose followed by the bursting of the hose. 74 SOLAS (14) II-2/10.2.3.1.1 17C

Resolution: Vessel crew replaced damaged hose with spare replacement onboard prior to completion of the exam. Damaged hose was destroyed in presence of PSCO.

Personal protective equipment shall consist of the following: protective clothing of material to protect the skin from the heat radiating from the fire and from burns and scalding by steam. The outer surface shall be water resistant.
