Firefighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO identified fixed CO2 nozzle beside L.O tank in the engine room covered in tape. Tape was removed on the spot. 74SOLAS(20) II-2/4.2.1.2

PSCO observed 20-30 cardboard pieces stored on E.R. deck besides active diesel oil leak. Additionally, PSCO observed several oil soaked rags stuffed throughout E.R. and purifier flat. 74SOLAS (20) II-2/4.1.3

A ship in which F/O is used, the arrangement for storage, distribution, and utilization of the fuel oil shall ensure the safety of ship and persons onboard. PSCO observed active diesel oil leak on bunker supply line valve in E.R. Leak was contained in a ZOL bucket which was actively overflowing in bilge. 74SOLAS(20) II-2/4.2.2

Resolution: Received class report attesting to replacement of the FTV41 valve on the fuel oil transfer and bunkering system. Pressure test carried out by crew on 21MAY24 with satisfactory result. During class attendance, no oil leaks were observed.

PSCO found leaks around hydraulic tank and pooled fluid on deck. Crew had removed fill pipe cape and modified piping arrangements resulting in spilled fluid on deck. 74SOLAS(04) II-1/29.12.1

A SECOND SOURCE OF ENERGY SHALL BE PROVIDED FOR AN ADDITIONAL THREE STARTS WITHIN 30 MIN UNLESS MANUAL STARTING CAN BE DEMONSTRATED TO BE EFFECTIVE. PSCO OBSERVED THE SECOND SOURCE OF POWER, FOR THE SHIP'S EMERGENCY GENERATOR, TO BE INOPERABLE.
