Satisfactory means (in the form of guard rails, … ) shall be provided for the protection of the crew in getting to and from their quarters, … and all other parts used in the necessary work of the ship. PSCOs observed multiple areas of chains replacing fixed railings on both the port and starboard main deck. The chain was excessively slacked to the point of rendering it ineffective as a guard rail. Additionally, PSCOs observed wasted and missing rails.

Resolution: Chains were tightened. Areas of missing/wasted railing were repaired on the spot by ships's fitter.
