Controlled tank venting systems fitted to tanks to be used for cargoes having a flashpoint not exceeding 60 degrees Celsius (closed-cup test) shall be provided with devices to prevent the passage of flame into the cargo tanks. : The materials of construction and the dimensions of gas sampling lines shall be such as to prevent restriction. PSCO observed 02 deck valves closed partially causing flow fault alarms for #6 port wing ballast tank and forward cleaning tank. 74SOLAS(09)II-2/5.7.2.3

- Electrical installation shall be such as to minimize the risk of fire and explosion from flammable products. PSCO observed multiple electrical conduit pipes in cargo deck area severely corroded and wrapped with duct tape not in compliance with hazardous location standards. These were not reported in most recent inspection in accordance with ships SMS. 74SOLAS(20)VII/10.1 IBC 10.1.2.1

Each vessel vapor connection flange face must have a permanent stud projecting outward that had a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between bolt holes, and in like with the bolthole pattern. PSCO observed starboard forward vapor flange missing vapor stud. 46CFR39.2001(j)

The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the provisions of the relevant rules and regulations established by the company. In meeting these requirements the company should ensure that: any non-conformity is reported with its possible cause if known the technical and or operational nature of the deficiencies noted provide objective evidence of a serious failure of implementation of the ISM code. An internal audit report satisfactory to the OCMI must be submitted prior to the clearance of this deficiency. 74SOLAS(20)IX/3.1 ISM CODE 10.2
