Two lifebuoys shall be provided with self-activating smoke signals complying with the requirements of the paragraph 2.1.3 of the code and be capable of quick release from the navigation bridge.
The casing of both smoke signals had cracks. The instructions on the signal had been painted over. The expiration dates on both signals were not visible and could not be verified. It took the crew 05 minutes to remove the smoke signal from the holder, demonstrating it could not be capable of quick release.

Resolution: Vessel submitted reciept of 02 smoke/light flares. Images were sent of the equipment installed and signed training records from the crew on use of the new equipment.
