Portable fuel systems, including portable tanks and related fuel lines and accessories, are prohibited except where used for portable dewatering pumps or outboard motor installations. Portable fuel tank was located in the engine room. Tank was removed from vessel.

Pre engineered fixed gas fire extinguishing systems are to be installed in accordance with the manufacturer's instructions. Fixed gas extinguisher bottle gauge found in "recharge" area. Repair or replace bottle. 46 CFR 181.420

The shut off valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space it must be located so that the operator does not have to reach more than 12 inches into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Fuel shutoff valves are located in the engine room more than 12 inches from the opening. 46CFR182.455(b)(4) 46CFR182.20-40(b)(3)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Vessels charts were not up to date. 46CFR184.420(a)(1)

A vessel must comply with the applicable requirements for any radio installation including the requirements for a radio log and a safety radio certificate to be issued by the FCC. Vessel has inoperable radio at main operating station. 46CFR184.502

A vessel must carry either a first aid kit approved under 160.041 or a kit with equivalent contents and instructions. Vessel had expired First Aid kit. 46CFR184.710

The employer shall provide an Employee Assistance Program (EAP) for all crewmembers. Vessel has not provided crewmembers with appropriate Drug and Alcohol training. 46CFR16.401

Each lifejacket carried on board the vessel must be marked in accordance with 185.604 of this chapter. Each lifejacket must be marked with Type I retroreflective material. Vessel had numerous lifejackets with substandard retroreflective material. 46CFR180.71(e) 46CFR185.604(h)

VHF-DSC radio did not have GPS installed.

4TH COLD MIL's COI expired on Mar 14th and the vessel operated with more than 6 passengers several times after the COI expired.

No proof of required EAP training. Provide evidence of EAP training.

Forward mast head light filled with water and inoperable. Fix or replace forward mast head light.
