All electrical apparatus shall be constructed and installed not to cause injury. The PSCO observed BWTS electrical panel altered from original state in a manner which could not avoid injury if power not secured. Crew members could not secure power from designated place.

Such search and rescue locating devices shall conform to the applicable performance standards not inferior to those adopted by the organization. PSCO observed 02 search and rescue locating devices with stenciled expiration dates. Upon further investigation the actual batteries are expired. When there are clear grounds for believing that the master or crew are not familiar with essential shipboard procedures relating to the safety of ships. Senior leadership informed PSCO on multiple occasions that the rescue boat engine can be operated without cooling water ships. SOLAS training manual for the engine states that for rescue boat operation the engine should not be operated without cooling water and in doing so may cause damage (Pg 12 and 17). 74 SOLAS (14) XI/4

The ships SOLAS training manual and engine manufacturer state that the water impeller should be replaced every 12 months. 01 - 1499 – The machinery, boilers and other pressure vessels, associated piping and fittings, shall be of a design and construction adequate for the service in which they are intended. The piping to the #2 sea water service pump suction inlet was found with an active leak due to deterioration of the piping and weld. Additionally, sea water piping associated with the M/E lube oil cooler was found with a repair made by the crew, which was not approved by the vessel’s classification society. The manner in which these were found increases the danger to all persons on board. SOLAS 2004 Cons. II-1/26.1; 17AC
