On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each space with a through-hull fitting below the deepest load waterline, such as a lazarette. While operating, the high water level alarm for the lazarette was plugged with paper towels in order to muffle a faulty nuisance alarm. The operator stated it was alarming despite no water in the bilge. Ensure proper operation of the high water bilge alarm prior to operations.

The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. Observed starboard engine power cable supported by plastic wire ties. Install with metal cable supports to the satisfaction of the attending Marine Inspector. 46 CFR 183.340(b)(4)

Vessel sustained significant damage to bow during collision with barge reported on 13 Jan 2019. Damage includes PORT and STBD bow bottom plates, keel, vertical stiffeners, section of collision bulkhead, PORT and STBD side plate, and bilge pump pipe. Vessel restricted from operations until repairs have been conducted and tested to satisfaction of marine inspector. In addition to repairs of hull plating, provide proof that engine mounts and mounting frames are operationally and structurally sound, perform shaft alignment, and provide proof that vessel is checked for fairness and meets ABS rules.
