Electrical installations shall be as such to minimize the risk of fire and explosion from flammable products. PSCO observed numerous light fixtures, motors, and electrical connections in the hazardous zone with open spliced wires, exposed wiring and damaged fixtures.

After any survey of the ship has been completed no change shall be made in the structural arrangements machinery, equipment & other items covered by the survey. PSCO observed modifications to inert gas system including patches on bottom of IGG system and zip ties around electrical connections of solenoid valves.

Before the ship leaves port & at all times during the voyage, all life saving appliances should be in working order and ready for immediate use. While testing the free-fall lifeboat engine. PSCO observed crewmembers unable to secure the engine following the test. Electrician replaced a faulty off switch.

The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the rules and regulations established by the company. The technical and operational nature of the deficiencies above provide objective evidence of a serious failure of the implementation of the ISM code. An internal audit report satisfactory to the OCMI must be submitted prior to clearing this deficiency.

Efficient guard rails or bulwarks shall be fitted to all exposed parts of the freeboard and superstructure decks. PSCO found Port Side guard rail in #4 port tanks warped and welds broken. Master stated damage occurred during Panama Canal crossing approximately 10APR2016.
