The purpose of this regulation is to prevent the ignition of combustion materials of flammable liquids. Means shall be provided to control leaks of flammable liquids. PSCO observed oil underneath the incinerator. RO must attest to the operation of incinerator prior to use. SOLAS 74(2009)II-2/4.1.1

MINIMUM SAFE MANNING CERTIFICATE SHALL BE IN COMPLIANCE WITH THE CONVENTION PRIOR TO THE MOVEMENT OF THE VESSEL. FLAG SHALL PROVIDE ATTESTATION OF THE CREW COMPETENCE AND FITNESS PRIOR TO VESSEL MOVEMENT. STCW 2017 I/2.11

Performance of crew members assigned fire-fighting duties shall be periodically evaluated by conducting on-board training and drills to identify areas in need of improvement, to ensure competency in fire-fighting skills is maintained, and to ensure the operational readiness of the fire-fighting organization. PSCO safety concerns and lack of crew competency while conducting fire drill in the emergency generator space 10JUL25. Upon re-inspection on 14JULY25, prior to the fire drill, crew members were fatigued and one member of the crew left the vessel for medical attention regarding heat exhaustion and dehydration. Flag to attest to crew training, completion of fire frills, and address unsatisfactory accommodation space. 74solas(09) II-2/1.5.2.2.3 STCW 2017 I/6 ILO/MLC 2006

The Company should establish procedures to ensure that the ship is maintained in conformity with the relevant rules and regulations and with any additional requirements which may be established by the company. External audit received by the Class Society BV noted 5 Major Nonconformities, an internal audit from the company shall be completed to address the appropriate corrective actions from all nonconformities.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs.: fire detection and alarm systems. PSCO observed a non-operative Hyper-mist system and fire detection in the purifier room. No spare were available. A Flag dispensation has been issued for the Hyper-mist and fire detection system for the vessel to sail to the next port in this condition with a manned space for the engine room.

Ships shall be providied with fire hoses, the number and diameter of which shall be to the satification of the administration. PSCO observed inadequate number of fire hoses in accordance with the fire controll plan. 74SOLAS(09)II-2/15.2.4.1

The Company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the Company. The technical and/or operational nature of the deficiencies noted provide evidence of serious failure of the implementation of the ISM Code. External audit received by the class society, BV noted 5 major nonconformities. An internal audit from the company shall be completed to address the appropiate corrective actions from all nonconformities. 74SOLAS(20)IX/3.1 ISM 10.1

The safety management system should include procedures ensuring that nonconformities, accidents and hazardous situations are reported to the Company, investigated and analyzed with the objective of improving safety and pollution prevention. PSCO observed that deficiencies 03-09 were not reported to Company. 74SOLAS2020 IX/3.1 ISM Code 9.1

The maintenance plan shall include at least the following fire protection systems and fire-fighting systems and appliances, where installed: fire mains. PSCO observed a temporary repair on the fire main and the vessel had no existing maintenance log for recording damaged equipment. SOLAS 2009 II-2 reg 14.2.2.3.1 17AC

Provision shall be made to facilitate cleaning, inspection and maintenance of main propulsion and auxiliary machinery including boilers and pressure vessels. PSCO observed the feed water piping being taken apart, shutting off hot water supply to the entire ship. SOLAS 2020 II-1/26.7 17AC

Spares and repair equipment shall be provided for life-saving appliances and their components which are subject to excessive wear or consumption and need to be replaced regularly. PSCO observed peeling and torn rescue quoits in the rescue boat. No spares were available. SOLAS 2020 III/20.5 17AC

In addition to the requirements contained elsewhere in the present regulations, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of the classification society which is reconized by the Administration in accordance with the provisions of regulation X1-1/1, or with applicable national standards of the Administration which provide an equivalent level of safety. PSCO observed a corroded junction box that was unable to open in way of an emergency shut-off switch for cargo crane operations. SOLAS 2009 II-1/3-1 40AC

The lifeboat engine shall be capable of operating for not less than 5 min after starting from cold with the lifeboat out of the water. PSCO observed that the lifeboat engine would not start. SOLAS 2020 III/31.1.2.1 LSA 4.4.6.3

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or the persons on board. PSCO's observed Main Engine #6 cylinder leaking water through the spacer o-ring. In addition, PSCO's observed lack of general cleanliness in the engine room. 74 SOLAS (2020) I/11(a)
