An atmospheric sampling point penetrated the steel bulkhead separating the boiler space and IGG space; the bulkhead is therefore not in compliance with Table 9.7 in SOLAS II-2/9 for proper fire protection.

Where main or auxiliary machinery including pressure vessels or any parts of such machinery are subject to internal pressure and may be subject to dangerous overpressure, means shall be provided where practicable to protect against such excessive pressure.
PSCO observed wire wrapped around safety valve handle of aux air receiver within the vicinity of start air compressors. Crew stated this was added because excessive vibration was causing unintended activation of the safety valve. Wire was removed and no unintended release happened.

PSCO observed auxiliary air receiver relief valve was wire tied to prevent the release of compressed air. Last internal inspection from SMS was 12 March 2020. There were no defect job reports made for this instance as required in ships procedures.

PSCO discovered that approximately 50 percent of the lifejackets on board the ship and internal buoyant material visibly showing due to the type of light that was inserted into the lifejacket.
