Electrical installations shall be such as to minimize the risk of fire and explosion from flammable products.
PSCO observed the pressurized enclosure for the TRO #1 had unsealed electrical penetration in the ballast treatment room. Several cargo pump motors were found with one or more bolts missing on the electrical enclosure, and the temperature sensor on the ROT pump had a missing wire and open electrical enclosure.

Oil tankers of 150 gross tonnage and above shall be equipped with an ODME approved by the administration. MEPC 108(49) requires that the current data be visibly displayed and the display is to include various data as listed in 6.10.
The vessel is fitted with a MEPC 108(49) ODME. The ODME display panel periodically displays various random signs in place of English letters and numerals.

Only one type of foam concentrate shall be supplied and it shall be effective for the maximum amount of cargos possible that are intended to be carried. The fire test for type A (alcohol resistant) foam is to be carried out with acetone and isopropyl alcohol.
The vessel is certificated to carry various cargoes that require type A (alcohol resistant) foam. The annual foam analysis dated 01/15/2023 does not indicate foam type and states the test was done with heptane (used for type B foam test).

IN ADDITION TO REQUIREMENTS IN PARAGRAPH 5.7.1 AND 5.1.2 OIL TANKERS OF 20,000 DWT AND ABOVE CONSTRUCTED ON OR AFTER 1JAN2012, SHALL BE PROVIDED WITH A FIXED HYDROCARBON DETECTION SYSTEM COMPLYING WITH FSS CODE FOR MEASURING HYDROCARBON GAS CONCENTRATIONS IN ALL BALLAST TANKS AND VOID SPACES OR DOUBLE HULL AND DOUBLE BOTTOM SPACES ADJACENT TO THE CARTO TAKES INCLUDING THE FOREPEAK TANK AND ANY OTHER TANKS AND SPACES UNDER THE BULKHEAD DECK ADJACENT TO THE CARGO TANKS.
THE GAS DETECTION SHALL BE SO DESIGNED THAT IT MAY READILY BE TESTED AND CALIBRATED.
OBSERVED THAT FIXED SYSTEM COULD NOT PROPERLY BE BUMP CHECKED AT FIXED GAS DETECTION PANEL & REPEATER, USING 50% LEL SPAN GAS. HIGHEST READING REGISTERED AT 12% LEL. DETECTOR UNIT CAPABLE OF READING 100% - LEL.
