External high pressure fuel delivery between the high pressure fuel pumps and fuel injectors shall be protected with a jacketed piping system capable of containing fuel from a high pressure fuel line failure. Resolution: Vessel replaced 01 fuel injector, replaced HP fuel oil lines on cylinder 2 and 5 and replaced the fuel pump on cylinder 3. Class attested to the repairs.

Resolution: Master stated that the crew turns the hyper mist off while conducting cargo ops to not accidently set off the system. The crew was notified that is not acceptable. The crew turned the hypermist system back online.

Every ship of 100 gross tonnage and above shall carry a garbage management plan which the crew shall follow. This plan shall provide written procedures for minimizing, collecting, storing, processing, and disposing of garbage. The vessels approved garbage storage locations are full. The vessel is storing garbage in the compactor room which has not been designated as a storage location per the garbage management plan. The compactor room is almost completely full of garbage and the room does not have fire detection or suppression installed within the space. MARPOL 73/78 Annex V 10.2 15C

The vessel reported the OCM as not operational, yet during a simulation of the 15 PPM alarm, everything seemed to work as it should. After examining the system, it was observed the over current protection circuit had loosened.
