All mechanistically propelled vessels of 1600 GT and over in ocean or coast-wise service must be fitted with a marine radar system for surface navigation. Vessel O/O reported their S-band radar inop. while transiting through Ambrose. Vessel O/O is to effect repair before resuming operations.

The company should establish procedures, plans, and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. Following the incident of June 02, 2025 where a member of the engineering department had been electrocuted while cleaning a strainer basket and based on our interview with crew members and assessment of the situation, the company is to conduct an audit of their Safety Management System to assess the effectiveness of current established procedures. Said audit is to be conducted by a third party ( Class). Corrective and mitigative actions are to be provided to OCMI as to prevent future recurrence of such incident.

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. A member of the engineering department has been electrocuted while conducting maintenance near electrical equipment. Safety procedures are to be implemented and followed to ensure safety of personnel. A risk assessment to cleaning SW/FW CW strainer baskets has been conducted by vessel crew describing the hazards of such activity and the control measures to effect such activity. That document or similar is to be made into a standard procedure to be followed by crew member in the maintenance of such strainer. Such deficiency is to be rectified prior to operations.
