The Oil Record Book Part 1 (ORB) shall be completed on each occasion, whenever any of the following machinery space operations takes place in the ship: discharge overboard or disposal otherwise of bilge water which has accumulated in machinery spaces. PSCO observed inconsistencies between oil record books, oil content meter records and engine room logs. During review, it was noted that on numerous occasions, the discharge of bilge water through the oily water separator was not accurately reflected between the ORB and the oil content meter log including, but not limited to the following dates: 25MAY26, 14MAY26, 30APR26, and 18FEB26. Additionally, the vessel’s sounding logs indicate significant changes in the tank levels for the oily bilge tank and bilge holding tank with no corresponding entries in the ORB or the oil content meter log including but not limited to the following dates: 22-23MAY26, 16-17MAY26 and 6-7MAY26. Paint lockers shall be protected by a carbon dioxide system, a dry powder system, a water-spraying or sprinkler system, or a system providing equivalent protection, as determined by the Administration. PSCO observed 19 cans (labeled 20-liters each) of paint stored in an enclosed space below the cargo compressor room, accessed from the port side of the ship, which is not designated as a paint locker and does not have an installed fire prevention system. 74 SOLAS (2024) II-2/10.6.3 17ac

As a minimum, [portable atmospheric testing instruments] shall be capable of measuring concentrations of oxygen, flammable gases or vapours, hydrogen sulphide and carbon monoxide prior to entry into enclosed spaces. Vessel was unable to provide enclosed space entry permits for entry into enclosed space below the cargo compressor room as required by their SMS. The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. PSCO observed leaks on the outboard, bottom side and forward, outboard side of the vessel’s sewage holding tank, resulting in sewage draining into the engine room and a dark stain forming outside the tank. 74 SOLAS (2024) I/11 17ac

The Oil Record Book Part I shall be kept in such a place as to be readily available for inspection at all reasonable times and, except in the case of unmanned ships under tow, shall be kept on board the ship.
