The company should ensure that all personnel involved in the company’s safety management system have an adequate understanding of relevant rules, regulations, codes and guidelines.
The operational nature of deficiencies 01, 02, & 05 provide objective evidence of lack of effectiveness in the implementation of the ISM Code. A safety management audit carried out by the Administration or the RO, with a corresponding report of findings satisfactory to the OCMI must be submitted within 30 days.
While loading cargo, vessel discharged oil through the ballast water system. The cause was determined to be a deteriorated lube oil drain pipe from the adjacent main engine cylinder lube oil storage tank 2(S), which passes through the aft peak tank. The ship’s SMS states; “Where ballast tanks adjacent to oil tanks or ballast tanks with oil pipelines running through them are fitted need to be checked for oil contamination. Ensure that the water in ballast tanks is uncontaminated prior to discharge, by sighting of the surface and sample drawn from the ballast tanks. Ballast water containing oil sheen on the surface must not be discharged.”
When PSCO questioned crew about the samples, crew members were unaware of the requirement per their SMS and stated they did not visually inspect or take a sample of the Aft Peak Tank prior to the discharge of ballast water. In reviewing the Ballast Water Record Book, no entries were made regarding visual inspection or sampling of the tank. Vessel also did not have 2 years of Ballast Records onboard as required by the Ballast Water Management Plan IAW their SMS.
