The machinery.....shall be of a design and construction adequate for the service for which they are intended and shall also be installed and protected as to reduce to a minimum any danger to persons on board.....and other hazards. The lock nut on the #1 & #5 cylinder solenoid valves backed off causing a lack of air flow to the main engine prohibiting the vessel from going astern.
