The Company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the code shall be treated as mandatory. In meeting these requirements, the Company should ensure that: .1.) Inspections are held at appropriate intervals. 2.) Any non-conformity is reported with its possible cause, if known. 3.) Appropriate corrective action is taken, and 4.) records of these activities are maintained. Upon inspection of the ship's maintenance records, it was found that the ship's general emergency alarm and public address systems have been inoperable since 04JAN2020. During the PSC exam on 27FEB2020, the Captain confirmed the non-conformity had not been reported and that no corrective action had been taken. 74SOLAS(14)IX/3.1 ISM CODE 10.2 30 a.b.c.

The Safety Management System should include procedures ensuring that non-conformities, accidents, and hazardous situations are reported to the Company, investigated, and analyzed with the objective of improving safety and pollution prevention. According to weekly maintenance records, both the general emergency alarm, and the public address system were inoperable since 04JAN2020 and there was no report made to the Company until 27FEB2020 while the PSC exam was being conducted. 74SOLAS(14)IX/3.1 ISM CODE 9.1 30 a.b.c.

Bilge wells in periodically unattended machinery spaces shall be located and monitored in such a way that the accumulation of liquids is detected at normal angles of trim and heel, and shall be large enough to accomodate easily the normal drainage during the unattended period. 04 bilge alarms in the main machinery space were tested. No audible alarms sounded in main control and no alarms registered on the engineer's alarm panel. 74SOLAS(14)II-1/48.1 17 a.

During the exam, PSCO discovered that the free fall lifeboat's steering system was initially inoperable. Crew was able to repair steering system prior to departure and the conclusion of the exam. 74SOLAS(14)III/20.2 17 a.
