A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A means to reset devices used to automatically shut down ventilation systems and engines. The vessel experienced a failure of engine restart after simulated activation of pre-engineered system during power trial.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A light to indicate discharge. Pre-engineered (Fireboy) system indicator light was inoperable.

Each small battery installation must be located in a well ventilated space and protected from falling objects. Battery in engine room was not covered and was not protected from falling objects.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with §80.59 once every five years. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Vessel doesn't have a Radiotelephony Certificate present. 47 CFR 80.903

The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. If posting is impracticable, such as in an open boat, the certificates must be kept on board in a weathertight container readily available for use by the crew and display to passengers and others on request. Certificate of Inspection was not available onboard the vessel.

When a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. If posting is impracticable, the stability letter must be kept on board in a weathertight container readily available for use by the crew and display to passengers. Stability Letter was not available onboard the vessel.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Vessel does not have an Emergency broadcast placard posted.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing actions to be taken in the event of fire, heavy weather, or man overboard conditions. Vessel did not have emergency instructions posted.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Vessel did not have servicing report for fixed fire extinguishing equipment.

The number and the identification for the items stowed inside, and their sizes must be marked in clearly legible letters and numbers on each container for life jackets. Number of lifejackets was worn out and not visible on stowage compartments.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel did not have instructions posted for the donning of lifejackets.

A power-driven vessel of less than 12 meters in length may exhibit an all-round white light and sidelights. The lights prescribed in these Rules (Subpart C) shall have an intensity as specified in Annex I to these Rules (33 CFR part 84), so as to be visible at the following minimum ranges:In a vessel of less than 12 meters:(v) A white, red, green or yellow all-round light, 2 miles; Vessel does not have proper all-around light. Vessel is limited to daytime operations only.

Ships must be provided with a VHF radio installation capable of transmitting and receiving DSC on frequency 156.525 MHz (channel 70) and it must be able to initiate the transmission of distress alerts on channel 70 from position from which the ship is normally navigated. Vessel's MMSI number is not programmed into VHF radio. 46 CFR 184.502 47 CFR 80.1085

A vessel must be provided with bilge pumps in accordance with Table 182.520(a). Vessel was missing a second bilge pump as required by 46 CFR 182.520(a).

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks.Marine Inspector found delamination on Fiber Reinforced Plastic (FRP) tabbing in the forward compartment along the collision bulkhead. Repair prior to credit drydock to the satisfaction of an attending USCG Marine Inspector. 705(c) Rectify prior to Credit Drydock. 46 CFR 176.802(a)(1)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Vessel did not have a light at the operating station to indicate when the automatic bilge pump is operating in the forward bilge compartment. 46 CFR 182.530 (c)

Vessels operating on limited coastwise routes must be provided with inflatable buoyant apparatus of an aggregate capacity that will accommodate at least 67% of the total number of persons permitted on board. Vessel did not have inflatable buoyant apparatus onboard. Vsl restricted to 3 NM during warm weather months and must be equipped with an EPIRB.

Provisions must be made closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. The vessel did not have a means for closing ventilation openings for the engine compartment. 46 CFR 182.465(h)

A vessel of not more than 19.8 meters (65 feet), carrying no more than 12 passengers, with machinery powered by diesel fuel and a fuel system built in accordance with ABYC H-33 (incorporated by reference; see 46 CFR 175.600), will be considered as meeting the requirements of this section. ABYC H-33.10.4; Require fuel tank connections and fittings shall be readily accessible, or accessible through an access panel, port or hatch. Term "readily accessible" is defined in H-33.4.3; Readily accessible - Capable of being reached quickly and safely for effective use under emergency conditions without the use of tools. Shutoff valves for fuel tanks are in the engine room and require a crew member to enter the space to operate the valves, thus not meeting the definition of "readily accessible". Provide plans that demonstrate vessels fuel system was built iaw ABYC H-33, specifically with the fuel shutoff valves or arrange vessel so fuel tank shutoff valves meet 46 CFR 182.455(b)(4).

A vessel that is exposed to salt water more than three months in any 12 mo period must undergo a drydock exam and internal exam at least once every two years. Last drydock was 19AUG16. Vessel is overdue for its drydock inspection. Vessel is restricted to its alternate services endorsements only (6-pack & recreational).

Retro reflective material was severely degraded on 75% of life jackets. Replace all retro reflective tape on life jackets prior to operation.

Update FCC radiotelephony certificate with correct call sign.

Protect all rotating machinery with suitable cover or guard.
