Vapor headers were not gas-freed, opened, and ready for internal inspection at the time of the inspection. Vapor piping systems that are used for polymerizing cargoes must be inspected internally at least annually. Gas free the cargo tanks and open the vapor headers for internal inspection prior to carrying polymerizing cargoes.

Resolution: All VCS piping was inspected and found to be clear.
