At each inspection for certification, the marine inspector may require that a man overboard drill be held under simulated emergency conditions specified by the inspector. Because the vessel has a COI endorsement to operate with a "Master Only" for operations lasting no greater than twelve (12) hours, a man overboard drill must be conducted where the master must recover a test dummy unassisted. The initial man overboard drill that was conducted on February 12, 2024, was failed and the vessel was issued a deficiency that required the vessel to operate with a full crew complement (01 MASTER and 01 DECKHAND) until USCG Inspectors witnessed a successful man overboard drill where the master recovers the test dummy unassisted. On February 22, 2024, when USCG Inspectors were onboard to witness a second attempt at performing a man overboard recovery drill using only the master, the master failed to recover the test dummy and stated that he did not have the proper equipment onboard to perform the recovery (boat hook). The vessel is prohibited from operating with a "Master Only", as endorsed on its COI, and must operate with full crew of (01 MASTER and 01 DECKHAND) until a successful man overboard drill is witnessed by USCG Inspectors.

Electrical equipment in spaces containing fuel tanks for gasoline must be part of an intrinsically safe system. Some coverings on electrical connections were damaged or missing. Ensure all electrical connections are properly covered and intrinsically safe.

Drill entries shall be made recording the date of the drill and general description. The vessel's log recording drills was missing. Replace log.

A vessel must carry a first aid kit approved by 46 CFR 160.041 or equivalent. First aid kit did not have proper approval. Place proper first aid kit onboard.

Remote fuel shut off valves must be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operation. Label fuel shut off valves.

The construction and arrangement of a vessel must give consideration to provisions of a seaworthy hull. Recent maintenance caused open wire/piping penetration holes to remain open between aft engineering space and fuel space. Close or plug unused openings.

The master was unable to recover the test dummy unassisted, but was able to complete the recovery with the assistance of the deckhand onboard. The vessel is prohibited from operating with a "Master Only", as endorsed on its COI, until a successful man overboard drill is witnessed by the attending USCG marine inspector under the operating conditions of "Master Only" (i.e. the master must make recovery unassisted).

Vessel must have a valid station license. Station license expired on February 6, 2024, and the new station license has not been received in the mail. Place new station license onboard vessel upon receipt.

A space containing fuel tanks for gasoline must have a ventilation system that complies with 46 CFR 182.460. Blowers for mechanical ventilation blowers were not operating. Repair blowers. Also, blower shutdown from installed firefighting system must be tested in presence of a USCG Marine Inspector after blowers are repaired prior to operating with passengers.

Where flexible nonmetallic hose is permitted for use in piping systems, it must meet SAE J-1942. Flexible non-metallic hose must be complete with factory-assembled end fittings on the hose or field attachable type fittings may be used. All hose end fittings must comply with SAE J-1475. If field attachable type fittings are used, they must be hydro-statically tested to twice the operating pressure of the system. Newly-installed fuel hoses are not J-1942 and are not attached with J-1475 fittings. Replace hoses with proper fittings prior to operating with passengers.

The general construction of the vessel must be such as to minimize fire hazards. Fuel piping from unused fuel tank is open-ended and not disconnected from tank. Isolate and disconnect all piping associated with unused fuel tank and ensure fuel tank is isolated prior to operating with passengers.

Each vessel must have a stability letter issued by the Coast Guard that sets forth conditions of operation. Current stability letter, issued 03 February 2014, states that no weights shall be added, removed, altered and/or relocated without authorization of cognizant [name withheld] installed vessel engines and decommissioning of forward fuel tank result in weight changes to the vessel. Either a new stability proof test must be conducted and new stability letter is issued by cognizant OCMI, or owner must prove that the new engines are equal in weight to the previous engines and the weight change of the empty fuel tank must be mitigated.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 and the owner must provide satisfactory evidence of required servicing to the marine inspector. Inspection of firefighting equipment is due.

Vessel must have at least three 2.5 gallon buckets with lanyard attached, labeled FIRE BUCKET. Vessel had only two fire buckets onboard. Place a third fire bucket onboard.

Cable and wiring must be supported with metal supports spaced not more than 24 inches apart and cable splicing must be insulated or protected. Wiring supports inadequate on back deck and machinery spaces and back deck wire splice was open and uninsulated. Repair and properly support wiring.

Vessels under 65' carrying less than 50 passengers need an hand bilge pump. Pump was missing. Place serviceable hand pump onboard.

The name of the vessel must be marked or painted in clearly legible letters on each life buoy. Mark life buoy with vessel name.

Each ring life buoy must be fitted with a lifeline of at least 60 feet in length. Attached lifeline is too short. Install lifeline of at least 60 feet.

Vessel must comply with applicable requirements issued by the FCC as set forth in 47 CFR 80. Vessel does not have a valid FCC Safety Radio Certificate. Obtain valid certificate.

A flammable vapor detection sensor must be located at the lowest part of a space containing a fuel tank. Repair or replace faulty vapor detection sensor and prove operation to the satisfaction of attending Marine Inspector prior to carrying passengers.

Vessel must provide evidence of annual fire fighting servicing for all fixed fire extinguishing systems. Provide proof of annual servicing for the fixed fire fighting system prior to carrying passengers.

A vessel must be provided with individual bilge lines and bilge suctions for each watertight compartment. Repair or replace bilge pump in aft compartment.

Vessel must have a means of accurately determining the amount of fuel in each tank by either a sounding pipe or installed marine type fuel gauge. Install a means to determine the fuel level.

Each battery used in lifesaving equipment must be replaced at the annual inspection. Replace battery in life ring buoy light.

The number of life jackets must be marked on containers used to store life jackets. Mark life jacket storage container with proper number.

Starboard exhaust fan inoperational. Repair or replace.

Submersible electric bilge pumps do not meet the requirements of UL 1113. Replace with approved bilge pumps.

Markings on Ring Life Buoy are faded and need to be remarked.

Ring life buoy floating light battery is expired and needs replaced. Replace and mark battery with installation date.

First aid kit is missing (25) aspirin. Replace as original.

Vessel equiped with only one of required two portable electric light. Obtain required second light.

No passenger permitted until a dry dock is completed.

Shaun Simoneaux and [name withheld] shall not operate in a 'master only' condition until satisfactorily conducting a Man-Overboard Drill in a reduced crew ('Master-Only') condition in the presence of the attending USCG Marine Inspector.

Replenish expired items in First Aid kit. 46 CFR 184.710
