12109 - Dangerous Goods - Goods of class 1 with the exception of division 1.4 shall not be stowed closer to the ship's side than a distance equal to one eighth of the beam or 2.4 m, whichever is less. PSCO observed 01 containers loaded with division 1.1 loaded in position 050984 which is within 2.4m of the side of the ship.

15104 - Safety Management Systems (ISM) - The company should clearly define and document the master's responsibility with regard to verifying that specified requirements are observed. PSCO observed C/O did not load cargo in accordance with Safety Management Manual 11.2 dangerous goods are shipped in accordance with international requirements, Deck and Cargo [name withheld] states IAW IMDG. SMS does not specify procedures for C/O for loading of dangerous goods using the ANKO or IMDG to ensure proper loading. Request an internal audit to be compeleted within 03 months.
