Each towing vessel must be fitted with a general alarm that is activated at each operating station and can notify persons onboard in the event of an emergency, is capable of notifying persons in any accommodation, work space, and the engine room, has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads: “Attention General Alarm—When Alarm Sounds or Flashes Go to Your Station” and A public-address (PA) system or other means of alerting all persons on the towing vessel may be used in lieu of the general alarm in paragraph (b) of this section if the system meets the requirements of paragraphs (b)(2) and (3) of this section. Vessel did not have a general alarm meeting paragraphs 2 and 3. 46CFR143.235

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: High bilge levels. 46CFR143.203

Lifejackets must have the following attachments and fittings: Each lifejacket must have a lifejacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the lifejacket. Lifejackets did not have the correct lights attached. 46CFR141.340

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light approved under approval series 161.010 or 161.110. Vessel only had 1 lifebuoy with a floating water life. 46CFR141.360

No later than July 22, 2019, the owner or managing operator must implement a health and safety plan. The health and safety plan must document compliance with this part and include recordkeeping procedures. Vessels H&S plan did not meet the requirements in 46CFR150.505-515. 46CFR140.500

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Flange was discovered exposed.

Life jackets must have the following attachments and fittings: (1) Each life jacket must have a life jacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the life jacket. (2) Each life jacket must have a whistle firmly secured by a cord to the life jacket.

If a towing vessel carries only one lifebuoy, the lifebuoy must be fitted with a floating electric water light approved under approval series 161.010 or 160.110, unless the towing vessel is limited to daytime operation, in which case no floating electric water light is required. Floating water light was found extinguished, replace battery.

Each remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it. Add instructions to fuel shut offs.

Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Replace worn lagging, and add additional lagging on turbo chargers.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Properly mount electrical outlet in galley space.

Twist-on type of connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. Properly splice connections in the engine room in the vicinity of main engines, and generator.

Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. Replace wing nut on battery outboard of generator.

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. Repair dog on aft deck cover.

The cognizant OCMI may require review of a vessel's watertight and weather tight integrity. This review may be performed by an individual who meets the requirements of § 144.140. Repair crack on STBD bulkhead, and hole from prior shore tie connection location.

Provide fire detection system that complies with 27.203 currently does not have circuit fault dectector

Provide fuel shutoffs operable from a safe place outside engine room. 46 CFR 27.207

Provide fixed fire fighting in accordance with 26 CFR 27.303 or approved B V semiporable fire-extinguishing system.

Repair fixed fire pump. 46 CFR 27.301

Repair fire hose holder to allow hose to be deployed without interference 46 CFR 27.301

Provide mounting brackets for B 1 fire extinguishers on Main Deck and wheelhouse. 46 CFR 25.30-20
