If the vessel's automatic identification system (AIS) radar, radio navigation receivers, gyrocompass, echo depth sounding device, or primary steering gear stops operating properly, the person directing the movement of the vessel must report or cause to be reported that it is not operating properly to the nearest Captain of the Port, District Commander, or, if participating in a Vessel Traffic Service, to the Vessel Traffic Center, as soon as possible. Vessel Master's notified Sector San Diego of an inoperableable S-Band radar and requested an Letter of Devitition (LOD). LOD was approved by the local OCMI and awaiting report from the Master of the vessel stating the repairs to the radar have been completed. 33 CFR 164.53

On March 22, 2023, your vessel reported a high temp alarm on the stern tube bearing and the vessel was required to raise the set point to avoid the component alarming. *The vessel is required to submit the manufacturer's report attesting to the parameters the equipment can safely operate in. *Additionally, the Chief Engineer must send in a attestation letter with his MMC# attesting to the temperature the stern tube bearing is operating at and that it is operating in accordance with the manufacturer's requirements. *The vessel must conduct and log hourly rounds while underway ensuring the bearing is not overheating and must notify this office if they have to change the set point again during their transit. *Finally, once the vessel arrives at their next port, the vessel must provide a technicians report attesting to the satisfactory repairs.

The condition of the ship shall be maintained to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons on board. The vessel's Captain reported fuel leaking from the #2 MOO storage tank. All fuel was transferred to another tank as a temporary solution, and the leaked fuel has been cleaned up. Vessel is to make permanent repairs prior to departure from the next port of call, Lyttelton, New Zealand, to the satisfaction of the attending class surveyor with corresponding report sent to USCG FEACT.

In meeting these requirements, the company should ensure that any non-conformity is reported, with its possible cause, if known [ISM 10.2.2]. Vessel failed to notify proper regulatory authorities (class, flag) as appropriate. Further, SMS documents PRO-VA-09, PRO-VA-10, and PRO HS 19 provided by company do not adequately address non-conformity reporting procedures. During MSP increased oversight inspection, Nonconformities were identified which resulted in issuance of 8 conditions of class. These non-conformities were known by vessel personnel but not reported to class or flag as appropriate. An internal SMS audit focusing on notification of nonconformities is required within 30 days.
