On March 22, 2023, your vessel reported a high temp alarm on the stern tube bearing and the vessel was required to raise the set point to avoid the component alarming. *The vessel is required to submit the manufacturer's report attesting to the parameters the equipment can safely operate in. *Additionally, the Chief Engineer must send in a attestation letter with his MMC# attesting to the temperature the stern tube bearing is operating at and that it is operating in accordance with the manufacturer's requirements. *The vessel must conduct and log hourly rounds while underway ensuring the bearing is not overheating and must notify this office if they have to change the set point again during their transit. *Finally, once the vessel arrives at their next port, the vessel must provide a technicians report attesting to the satisfactory repairs.

The vessel's Captain reported fuel leaking from the #2 MOO storage tank. All fuel was transferred to another tank as a temporary solution, and the leaked fuel has been cleaned up. Vessel is to make permanent repairs prior to departure from the next port of call, Lyttelton, New Zealand, to the satisfaction of the attending class surveyor with corresponding report sent to USCG FEACT.

Muster and embarkation shall be adequately illuminated by lighting supplied from the emergency source of electrical power. Embarkation lighting on rescue boat deck was inop. Repair to the satisfaction of the attending MI. 74 SOLAS 20 CONS. (III/11.4)

Fire fighting appliances shall be kept in good working order and readily available for immediate use. Emergency fire pump was found leaking at the flange and seal. Repair to the satisfaction of the attending MI. 74 SOLAS 09 CONS. (II-2/14.2.1.2)

Emergency lighting located at the fire pump must last 18 hours. Lighting at FP was inoperable. Repair to the satisfaction of the attending MI. 74 SOLAS 09 CONS. (II-1/43.2.2.6)

Muster lists and emergency instructions complying with the requirements of regulation 37 shall be exhibited in conspicuous places throughout the ship. General alarm placard was not permanently posted in E/R. Affix placard to the satisfaction of the attending MI. 74 SOLAS 20 CONS. (III/8.3)

Containers, brackets, racks, and other stowage locations for life-saving equip shall be marked w/ symbols IAW recommendations of the Organization, indicating the devices stowed in that location for that purpose. IMO symbol markings were missing from multiple life ring locations on deck. Replace to the satisfaction of the attending MI. 74 SOLAS 20 CONS. (III/20.2)

Fire fighting appliances shall be kept in good working order and readily available for immediate use. Fire damper located at the incinerator room was unable to close. Repair to the satisfaction of the attending MI prior to using incinerator. 74 SOLAS 09 CONS. (II-2/14.2.1.2)

Vessel was found with unauthorized weld repair. Provide documentation of RO approval of weld repairs, IWO stbd side potable water tank at the transom bulkhead, to the attending MI. 74 SOLAS 20 CONS. The company and the ship shall comply with the requirements of the ISM Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. In meeting these requirements, the company should ensure that any non-conformity is reported, with its possible cause, if known [ISM 10.2.2]. Vessel failed to notify proper regulatory authorities (class, flag) as appropriate. Further, SMS documents PRO-VA-09, PRO-VA-10, and PRO HS 19 provided by company do not adequately address non-conformity reporting procedures. During MSP increased oversight inspection, Nonconformities were identified which resulted in issuance of 8 conditions of class. These non-conformities were known by vessel personnel but not reported to class or flag as appropriate. An internal SMS audit focusing on notification of nonconformities is required within 30 days.

The following functional requirements shall be met in regards to maintaining the effectiveness of fire safety systems on board: Fire protection systems and fire-righting systems shall be maintained and ready for use. Several fire hose boxes(containing all hoses and all necessary fittings and tools) throughout the vessel are unservicable and require repair/replacement.

The fire resistance of doors shall be equivalent to that of the division in which they are fitted. Door leading down to the engine room from the passageway (a class boundary) was missing a lower hinge and the door was loose.

Doors required to be self-closing, shall not be fitted with hold back hooks. Inside the galley was a door held open by a bungie cord (A class boundary).

Electrical installations shall be such that the safety of crew from electrical hazards will be ensured. A new transformer in the electricians shop was replaced after another casualty and had a hole open to the electrical equipment inside.

Electrical and hydraulic systems shall be maintained from the emergency switchboards at all times. The battery cables on both banks of the emergency start batteries were loose at the lugs and posts.

Electrical installations shall be such that the safety of crew from electrical hazards will be ensured. Several light fixtures throughout are damaged and require repair/replacement.

The machinery shall be of a design and construction adequate for its intended service. Vessel experienced loss of power and loss of propulsion due to unknown reasons. Power overload is suspect while bringing bow thruster online. DNV-GL to make assessment and certify proper operation of subject machinery.

The means for securing and maintaining weathertightness shall be to the satisfaction of the administration. The arrangements shall be to ensure that the tightness shall be maintained in any sea condition. During inspection daylight was observed in the athwartship seals between all hatch panels on both cargo holds. Class DNV GL surveyor in attendance wrote repair condition to be corrected by 05MAR2020. Vessel to ensure all hatch seams are weathertight prior to sailing.

Vessel experienced multipule punctures and damage in the forward ballast tank adjacent to the number one cargo hold when a heavy lift spreader bar broke loose in heavy weather. Make repairs as per the proposed repair plan. Repairs required to be made by 18DEC2019 as per CG 835 and DNV-GL Condition of class.

The arrangement of fire pumps shall be as to ensure that if a fire in any one compartment could put all the pumps out of action, there shall be an alternative means consisting of an emergency fire pump with its source of power and sea connection located outside the space where the main fire pumps are located. Emergency fire pump is currently inoperable due to broken coupling from motor to pump. 74/78 SOLAS (2009 Consolidated) II-2/Regulation 10.2.2.3.1.2

Class survey identified multiple existing material deficiencies which indicate failure of Safety Management System implementation. By 19AUG2017 the Company shall provide OCMI with report of corrective action follow up and Recognized Organization actions.

Recommend external ISM audit after objective evidence was found in various systems & components throughout the ship to include;life saving equipment maintenance, self closing doors, flammable liquids and oxidizers stored in non approved lockers (Sauna), record & maintenance keeping after computers were infected with virus & had to be replaced, failure to report equip malfunction and missing annual

Until periodic safety procedures are proven to the satisfaction of Class or CG, engine room shall be manned using both licensed and unlicensed engineers on a 4 hour watch schedule

Loop #14 of ships fixed fire detection system (servicing grommet lockers and a storage locker between hatches 1 & 2) is inoperable. Make necessary repairs to fixed fire detection installation to ensure full functionality of system to satisfaction of attending Class Surveyor or Coast Guard Marine Inspector.

Main propulsion machinery must be equivalent to the standards established by Class. Fuel injection pumps 1, 2, 5 and 6 failed and were repaired by crew on board. Class to attend and attest to repairs.

Show proper operation of remote F/O sutoff for emergency generator.

Vessel is 23 days outside of its 3 month window for completing its annual inspection

Vessel missing General Alarm placards in all spaces. Provide proof of compliance with 46 CFR part 97 as required by NVIC 01-13.

In the Steering Gear Flat, remove and clean the compressor oil from the deck. Properly dispose of 2 x 5 gallon leaking pails.

In the Incinerator Room reinstall the removed deck grating.

In the Incinerator Room, relocate the Diesel Oil Tank vent to outside the space and install a proper flame screen. (The current installation of the independent diesel oil tank for the incinerator has no flame screen fitted to the tank vent pipe, which terminates inside the Incinerator space.)

Properly mount the 6-man life raft forward.

In the Paint Locker, mount the 6 Kg hand Portable Fire Extinguisher.

Secure all loose gear in the Forecastle.

Provide non-conductive electrical matting for the switch gear at numerous locations.

Reinstall the water level alarm in Room 128.

In the Crew Laundry Room, properly install the dryer vent hose to the vent duct to the house exhaust vent duct.

Apply the Official Number to a main beam of the vessel.
