: Oil sludge tanks shall be provided and shall be of adequate capacity, having regard to the type of machinery and the length of voyage, to receive the oil residues which cannot be dealt with otherwise in accordance with the requirements of MARPOL Annex 1. Vessels incinerator was found to be inoperable due to deteriorated refractory. In addition, the crew is currently using 4 tanks, outside of those approved on the IOPP Form A, for the storage of sludge indicating inadequate storage capacity for the machinery and intended/previous voyages. PSCOs identified the following unauthorized tanks being used; #6 Fuel Oil Tank, Cofferdam between M/E Lube Oil Sump & Bilge Holding Tank, M/E Lube Oil Settling Tank and Generator Lube Oil Settling Tank

PSCOs observed fuel oil leaks on all three generators and the generator recirculation fuel oil pump.

The condition of the ship and its equipment shall be maintained to conform to the provisions of the present regulation to ensure that the ship in all respects will remain fir to proceed to see without danger to the ship or persons on board. PSCOs observed that the ships incinerator was inoperable due to deteriorated refractory. The crew provided documentation attesting to company notification in February with no rectifications taking place.

The company and the ship shall comply with the requirements of the International Safety Management code. For the purpose of this regulation, the requirements of the code shall be treated as mandatory. The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel. As noted in deficiencies 01, 03, and 06 the PSCO believes there is objective evidence that the ship is not being maintained or operated as required in relation to sludge storage, use of incinerator and insufficient potable waster for the crew.
