An immersion suit of an appropriate size complying with the requirements of section 2.3 of the code shall be provided for every person on board the ship. PSCO observed crew members were unable to don complete immersion suits within two minutes.

All life-saving appliances and arrangements shall comply with the applicable requirements of the code. The liferaft shall be packed in a container that is as far as practicable watertight. The davit launch liferaft is not watertight.

Each liferaft shall be stowed with a float-free arrangement complying with the requirements of paragraph 4.1.6 of the code so that each floats free and inflates automatically when the ship sinks. Hydrostatic release units were installed incorrectly.
