During an ACP Annual inspection, an expanded Safety Management System (SMS) examination revealed objective evidence of a systematic breakdown in the implementation of the vessel's planned maintenance system. Specifically, the vessel's bilge system—including bilge wells, piping, isolation valves, and the primary bilge pump—has not been inspected, tested, or maintained in accordance with the procedures, intervals, or checklists required by the company's approved Safety Management System (SMS).

During a physical inspection of the vessel's dewatering systems, the primary bilge pumping system was found inoperable and not working as designed. Specifically, the main bilge pump failed to hold a prime or draw vacuum, preventing the evacuation of accumulated water from the bilge wells.

Ventilation ducts shall be provided with fire dampers capable of being closed from outside the space served, and arranged so as to maintain the fire integrity of divisions. IM observed fire dampers for the engine room on the second deck not closing as designed.

During a safety inspection of the steering gear compartment, the emergency escape watertight hatch leading to the main deck was found defective and could not close as designed.

Each power sliding operated watertight door shall have an approximately uniform rate of closure under power and in no case be less than 20 seconds or more than 40 seconds. MI observed all Class III doors closing less than required 20 seconds.

During an examination of the vessel's statutory pollution prevention records, it was discovered that a recent fuel bunkering operation was not recorded in the Oil Record Book (ORB) Part I. Specifically, a cross-reference of the vessel's Bunker Delivery Notes (BDNs) and the deck log indicated that the vessel received fuel oil on 14July2026; however, there is no corresponding entry made in the ORB under Code 'H' (Bunkering of fuel or bulk lubricating oil) documenting the date, time, port, quantities, and tanks utilized during this transfer. Chief Engineer shall correct the ORB to accurately document the bunkering operation, and the corrected entry shall be verified.

During a safety inspection of the vessel's emergency communication systems, the forward Public Address (PA) system speaker located on the topside forecastle deck was found inoperable.

The automatic pre-discharge alarm loop for the NOVEC fixed fire-extinguishing system is defective. When the remote control boxes on both the navigation bridge and the main deck were opened, the micro switches failed to actuate, and no pre-discharge alarm sounded or flashed inside the protected machinery space.

An efficient bilge pumping system shall be provided, capable of pumping from and draining any water-tight compartment. MI observed failed test of bilge system.

Means of control shall be provided for openings and closure of daylights, closure of openings in funnels which normally allow exhaust ventilation and closure of ventilator dampeners. Mi observed dampeners not closing.

Opening in watertight bulkheads. MI observed class III sliding WTD in the ER not sealing. Gasket was painted and deformed causing door not to seal.

An alarm system complying with regulation 51 shall be provided for all important pressures, temperatures, and fluid levels and other essential parameters. Observed STBD Main Diesel Engine Low Lube Oil Level alarm was programmed in the blocked state preventing from being activated in the case of a low lube oil level on the STBD engine. 74 SOLAS 2020 II-1/53.4.3

A Chemical tanker shall comply with the requirements of the International Bulk Chemical Code and shall, in addition to the requirement of regulation I/8, I/9, and I/10 as applicable, be surveyed and certified as provided for in that Code. An International Certificate of Fitness for the Carriage of Dangerous Chemicals in Bulk shall be issued for a period specified by the Administration which shall not exceed 5 years. Observed vessel did not have a valid Certificate of Fitness for the carriage of Noxious Liquid Substances. 74 SOLAS 2020 VII/10.1 IBC Code 1.5.6.1

Maintenance, testing and inspections shall be carried out based on the guidelines developed by the organization and in a manner having due regard to ensuring the reliability of the firefighting systems and appliances. Observed there was no maintenance being conducted on the SCBA Compressor. 74 SOLAS 2020 II-2/14.2.2.1

The company and the ship shall comply with the requirements of the international safety management code. For the purpose of this regulation, the requirements of the code shall be treated as mandatory. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements with maybe established by the company. Vessel personnel could not provide maintenance records or procedures for SCBA air compressor. Company shall provide Non conformity and corrective action plan within 30 days. 74 SOLAS IX-3.1 ISM Code 10.1

The training manual, which may comprise several volumes, shall contain instructions and information, in easily understood terms illustrated where ever possible. Observed SOLAS training manual not reflecting current Lifesaving /firesaving equipment onboard. 74 SOLAS 2020 III/35.3

Doors provides to ensure the water tight integrity of internal openings which are used while at sea are to be sliding water tight doors capable of being remotely closed from the bridge and are also to be operable locally from each side of the bulkhead. Indicators are to be provided at the control position showing whether the doors are open or closed, and an audible alarm is to be provided at the door closure. Observed type 3 water tight door leading to steering compartment did not have an audible alarm. 74SOLAS2020 2-I/13-1.2

Provisions shall be made to facilitate clean, inspection and maintenance of main propulsion and auxiliary machinery including boilers and pressures vessels. Observed excessive oil leak on starboard MDE front seal. This oil leak was identified in HELM on 06/13/2021 (477 days) Oil leak shall be corrected at next dry dock (12/2022). 74SOLAS2020 2-I/26.7

All life-saving appliances and arrangements shall comply with the applicable requirements of the code.Where davit arms are recovered by power, safety devices shall be fitted which will automatically cut off the power before the Davit arms reach the stops in order to prevent overstressing the falls or Davits, unless the motor is designed to prevent such overstressing. Observed limit switch on davit fall not working as designed. 74 SOLAS 2020 III/34 LSA Code 6.1.2.7

Except as provided by 126.120 and 126.260, no OSV may be operated without a valid COI. Vessel is conducting operations under an expired COI. Vessel COI expired on 26 SEP 2022. 46 CFR 126.210

Maintenance, testing and inspections of life saving appliances shall be carried out in a manner having due regard to ensuring reliability of such appliances. Rescue boat davit was not serviced as per manufactures recommendation. MI found all grease fittings painted/rusted over. Master shall create a non conformity and corrective action as per the SMS. 74SOLAS2020 III/20.3.1

Before the ship leaves port and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate use. MI found rescue boat righting lines missing.74SOLAS2020 III/20.2

The following fire protection systems shall be kept in good order so as t ensure their required performance if a fire occurs: Structural fire protection, including fire resisting divisions, and protection of openings and penetrations in these divisions. MI found galley A60 steel rolling fire door not serviced as per manufactures recommendation. 74SOLAS2020 II-2/14.2.1.1.1

Means of control shall be provided for opening and closure of skylights, closure of openings in funnels which normally allow exhaust ventilation and closure of ventilator dampeners. MI observed stack fire dampeners not closing correctly.74SOLAS2020 II-2/5.2.2.1

Access doors and access hatch covers normally closed at sea, intended to ensure the watertight integrity of internal openings, shall be provided with means of indication locally and on the bridge showing whether theses doors or hatch covers are open or closed. A notice is to be affixed to each such door or hatch cover to the effect that is not to be left open. Steering gear escape hatch indicator switch found not operating as intended. 74SOLAS2020 II-1 13-1.3

Hatchways closed by weather-tight covers of steel or other equivalent material fitted with gaskets and clamping devices. MI found bow thruster hatch gasket not making a complete seal. loadline chp 1 reg 16

All watertight doors and hatches shall be watertight. The aft steering watertight door, vestibule engine room watertight door, and main deck escape hatch were unable to seal properly and dogs need to be adjusted. The aft steering room and bow thruster room escape hatch gaskets were unable seal properly and need to be replaced.
