During an ACP Annual inspection, an expanded Safety Management System (SMS) examination revealed objective evidence of a systematic breakdown in the implementation of the vessel's planned maintenance system. Specifically, the vessel's bilge system—including bilge wells, piping, isolation valves, and the primary bilge pump—has not been inspected, tested, or maintained in accordance with the procedures, intervals, or checklists required by the company's approved Safety Management System (SMS).

During an examination of the vessel's statutory pollution prevention records, it was discovered that a recent fuel bunkering operation was not recorded in the Oil Record Book (ORB) Part I. Specifically, a cross-reference of the vessel's Bunker Delivery Notes (BDNs) and the deck log indicated that the vessel received fuel oil on 14July2026; however, there is no corresponding entry made in the ORB under Code 'H' (Bunkering of fuel or bulk lubricating oil) documenting the date, time, port, quantities, and tanks utilized during this transfer. Chief Engineer shall correct the ORB to accurately document the bunkering operation, and the corrected entry shall be verified.

The automatic pre-discharge alarm loop for the NOVEC fixed fire-extinguishing system is defective. When the remote control boxes on both the navigation bridge and the main deck were opened, the micro switches failed to actuate, and no pre-discharge alarm sounded or flashed inside the protected machinery space.

Indicators are to be provided at the control position showing whether the doors are open or closed, and an audible alarm is to be provided at the door closure. All watertight doors and hatches shall be watertight. The aft steering watertight door, vestibule engine room watertight door, and main deck escape hatch were unable to seal properly and dogs need to be adjusted. The aft steering room and bow thruster room escape hatch gaskets were unable seal properly and need to be replaced.

Resolution: Vessel provided photographic evidence that new gaskest were installed and new part for the dogs were installed to allow all watertight doors and hatches to seal properly and be watertight.
