Due to the repeated failure of the main engine starting air distributor system and related components evident in previous Activity # 7890256 and below, an external audit report satisfactory to the OCMI must be submitted prior to clearance of this deficiency.

Vessel reported abnormal main engine vibrations during a pre-arrival check engine astern test. The main engine did not start astern after stopping the vessel and conducting further assessment while enroute to the Port of New York and New Jersey.
