07114-Remote Means of control(opening,pumps,ventilation,etc.) Machinery spaces-Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500L and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. PSCO observed DO storage tank quick acting closing valve was blocked. Proper operation of the valve shall be tested. 74 SOLAS (a) 2014 Cons. II-2/4.2.2.3.4 60 A/C

Resolution: Received, reviewed and accepted class report from DNVGL stating the deficiency was rectified. Surveyor found that the valve would not stay open due to missing lock set screws for the valve stem limiter spacer. Valve was repaired and tested satisfactorily.

05111-Satellite EPIRB 406MHz/1.6GHz-Subject to the provisions of regulation 8..3 a satellite emergency position-indicating radio beacon (EPIRB) which shall be capable of floating free is the ship sinks and of being automatically activated when afloat. PSCO observed a life raft hydrostatic release attached to the EPIRB which stated, “Hydrostatic release unit for life rafts of all capacities”. Crew could not find any documentation for the HRU verifying it could be used on an EPIRB. 74SOLAS (a) 2014 Cons. IV/7.6.6.4 17 C

The Chief Engineer provided maintenance records indicating that all quick-closing valves were tested on 18NOV2019, however upon further questioning stated that the valve referenced in Deficiency #1 had not been tested since he joined the vessel in August. An internal SMS audit is required to be completed within 03 months. 74 SOLAS (a) 2014 Cons IX/3.1, ISM Code 6.4
