At least two lifebuoys, each with attached self-activatiing smoke signals, must be stowed where they can be quickly released from the navigating bridge and should, when released, fall directly into the water without striking any part of the vessel. Vessel is missing bridge buoys.

Except as provided by this section, no OSV of less than 6,000 GT ITC (500 GRT if GT ITC is not assigned) may carry a noxious liquid substance (NLS) in bulk without the approval of the Commandant (CG-ENG). The vessel’s Certificate of Inspection list cargoes (i.e. methanol)that require compliance with the IBC Code / CG ENG Policy Letter No. 03-12. CH-1. This includes cargo tank relief valves with a 2.9 PSI pressure setting on tanks in which such cargoes are carried. The relief valves are on board but not installed. A class 1 door must be designed to withstand a head of water pressure. Watertight door in engineroom leading into the escape route had air sucking into the engine room and a loose dog.

Each life ring must be marked with retro reflective material. Most life rings on board had weathered and torn retro reflective tape.
