THE MACHINERY AND FITNESS SHALL BE OF A DESIGN AND CONSTUCTION ADEQUATE FOR THE SERVICE FOR WHICH THEY ARE ARE INTENDED AND SHALL BE INSTALLED AND PROTECTED AS TO REDUCE TO A MINIMUM ANY DANGER TO PERSONS ON BOARD. SHIPYARD REPAIRS TO INCINERATOR WHILE IN DRY DOCK AND HAD APPLIED COATINGS TO REPAIR. THE INCINERATOR IS AGAIN DETERIORATED AND AND DAMAGED. THE REFRACTORY BRICKS ARE SEVERLY DAMAGED AND AREAS EXPOSED. VESSEL HAS ALREADY ORDERED AND RECIEVED THE REFRACTORY TO MAKE REPAIRS.

Resolution: Vessel crew replaced fire gasket that was too thick with original equipment which allowed the door to close.
