Old light boxes on main deck aft missing covers with exposed dead end wires and power supplied in hazardous deck space area. Have boxes/wires covered to explosion/intrinsically safe type fixture or remove to satisfaction of RO prior to carriage of hazardous cargoes. SOLAS(20) II-1/45.5.4

Cargo tanks shall be fitted with a visual and audible high-level alarm which complies with 15.19.1 to 15.19.5 and which indicates when the liquid level in the cargo tank approaches the normal full condition. #2 starboard liquid mud tank high-level alarm was disarmed by engineers do to issues with constant sounding. Have tank level indication installed prior to loading NLS in tank to the satisfaction of the RO. MARPOL Annex II/11.2 IBC Code 15.19.6

Bunker delivery notes issued in US gallons but converted to cubic meters equaling improper values causing tracking issues. Have amounts corrected and standard measuring implemented for use by all C/E's to the satisfaction of the RO. MARPOL Annex I/17.4
