All the steering gear components and rudder stock shall be of sound and reliable construction to the satisfaction of the Administration. PSCO observed that during steering gear test of pump #2, steering gear #1 automatically turned on, two different times. Furthermore, steering gear #1 lost power to the control panel. PSCO conducted a second test to verify operation. PSCO observed steering gear pump number 1 experienced a Common Alarm and pump #2 started automatically.

Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling, or daily service tank having a capacity of 500l and above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which the tanks are situated. PSCO observed emergency fuel shut off valve to the ermergency generator inoperable.
