For the purpose, the following functional requirements shall be met: (.2) means shall be provided to limit the accumulation of flammable vapors. PSCO observed excessive amount of oil around the tops of the engine cylinders due to loose fuel line connections and maintenance.

PSCO observed SFP(insulation tape) was missing, torn, and peeling away in numerous locations throughout emergency escape trunk in engine room.

PSCO observed the access cover to the bilge tank actively leaking. Vessel had a flag dispensation for holding sewage in the slop tanks, however, sewage was draining to the bilge tank. 74 SOLAS (20) I/11(A)
