46 CFR 115.804(c) (a)At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following:(c) Inspection of all machinery essential to the routine operation of the vessel including generators and cooling systems; Marine Inspector observed burn marks and charring of the port generator and upon closer inspection, identified a hole in the crankcase. Objective evidence indicated there was a crankcase explosion and potential fire occurred. Marine Safety Unit St. Thomas did not receive notification of any marine casualty prior to this inspection. The crew was unable to provide any definitive answers surrounding the hole in the crankcase. Provide repair proposal plan to local MSU St. [name withheld] in Charge, Marine Inspection for review and approval prior to conducting repairs.

46 CFR 115.810(a)(2) (a)At each initial and subsequent inspection for certification, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following:(2) Inspection of piping, controls, and valves, and the inspection and testing of alarms and ventilation shutdowns, for each fixed gas fire extinguishing system and detecting system to determine that the system is in operating condition. Marine Inspector observed a visual alarm in the wheelhouse for the fixed fire-fighting system, indicating the engine room pre-engineered fire supression system was not in full operational status. After further inspection, Marine Inspector observed unauthorized solder repairs to the Fireboy system. After questioning the crew, a crew member stated he took the Fireboy bottle home to make solder repairs because a fire servicing technician was not available. Marine Safety Unit St. Thomas did not receive notification of any marine casualty prior to this inspection. Provide third party fire servicing report to the MSU St. [name withheld] in Charge, Marine Inspection.

(a) An inspection is required before the issuance of a Certificate of Inspection. Such an inspection for certification is not made until after receipt of the application for inspection required by § 115.105 of this part. (b) Upon receipt of a written application for inspection, the cognizant OCMI assigns a marine inspector to inspect the vessel for compliance with this subchapter at a time and place mutually agreed upon by the OCMI and the owner, managing operator, or representative thereof. (c) The owner, managing operator, or a representative thereof shall be present during the inspection. Vsl COI lapsed and is now expired. Vsl owner/operator shall request inspection date with local Marine Inspectors and conduct satisfactory COI inspection prior to operating. 46 CFR 115.400(a)(b)(c)

A self-propelled vessel must meet the applicable requirements for main and auxiliary steering apparatus in subchapters F (Marine Engineering) and J (Electrical Engineering) of this chapter. The main steering gear and rudder stock must be Capable of moving the rudder from 35° on either side to 35° on the other with the vessel at its deepest loadline draft and running at maximum ahead service speed, and from 35° on either side to 30° on the other in not more than 28 seconds under the same conditions; operated by power when necessary to comply with paragraph (b)(2) of this section. Loss of steering on port rudder was reported. Determine root cause for loss of steering on port rudder and make necessary repairs to the satisfaction of the attending marine inspector.

A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. 01 missing child lifejacket from the designated storage location on the upper PAX deck. Replace as required. 46 CFR 117.71(b)

An adult life jacket must be provided for each person carried on board a vessel. MI’s observed 09 lifejackets on the upper PAX deck that were no longer serviceable.Replace as required. 46 CFR 117.71(a)

Electrical equipment on a vessel must be installed and maintained to protect crew from electrical hazards. Bilge manifold compartment missing lights and covers. 46 CFR 183.200

The following spaces must be equipped with fire detection and alarm systems of an approved type that is installed in accordance with part 76 in sub-chapter H of this chapter. Accommodation spaces, propulsion machinery spaces and control spaces. Attending MI's noted that the fire detection panel in the wheelhouse had active faults. When two smoke detectors where tested in the accommodation space no alarm sounded on the bridge. Prior to operations with passengers, clear all faults and restore the system to full operations.

Each vessel must be provided with a visual and audible alarm at the operating station to indicate a high water level in each of the following unmanned spaces: a space with a through hull fitting below the water line, machinery space, bilge well, shaft alley. Attending MI's noted that a nuisance alarm was continuously sounding when the bilge high water alarm panel was energized. System is to be restored to full operational capacity prior to departure.

A vessel must comply with the applicable requirements for any radio and EPIRB installations as set forth in 47CFR part 80. Attending MI's noted that the EPRIB annual certification was past due.

A vessel that is exposed to saltwater more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. The RED HOOK I is due for drydock examination no later than June 30, 2021. The vessel must undergo a credit drydock examination to the satisfaction of the USCG prior to carrying passengers for hire.

42 adult lifejackets removed from service due to sun-damaged torn fabric. Replace 42 adult lifejackets. Vessel passenger count limited to 161 until lifejackets are replaced.

Demonstrate bilge suction from forepeak and lazarette prior to receiving annual inspection credit.

Provide a correct written passenger count log.

Provided updated/ current Coast Pilot [name withheld], and Charts.

Provide updated drill log entries for man overboard, fire and abandon ship drills.

Vessel log book missing due to hurricane damages.

PFDs not stored in convenient locations distributed evenly throughout the vessel. Most PFD's for upper deck were not in their location, but stored in a locker on main deck.

PFDs were not stowed in accordance with approved lifesaving equipment plan.

The vessel master was unaware of the location of the PFDs relocated form the storage locations on the upper deck.

The locker which the PFDs had been moved to was not marked with PFD quantities inside.

Determine cause of fire alarm panel faults and show proper operation.

Provide current navigation publications.

Replace grab line on upper deck aft ring buoy.

Replace articulating fitting on port fwd compartment remote operated valve.

Replace stbd fire/bilge suction valve and port generator suction valve.
