To assist in casualty investigation ships, when engaged on international voyages, subject to the provisions of Regulation 1.4, shall be fitted with a voyage data recorder, if, a ship is other than a passenger ship of 3000 gross tonnage and upwards constructed on or after 01July2002. The Port State Control Officer observed the VDR panel showing the following error "Float Free Issue- Float Free Cannot Find" The Captain and on board technician cannot attest to the proper operation of the VDR.

Resolution: Class issued a short term Cargo Ship Safety Equipment Certificate to expire on 03OCT25. Repairs to be made when parts are delivered onboard in Corpus Christi.
The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Vessel was in collision with small pleasure craft in shipping channel. Class survey required to ensure no damage to vessel hull and water tight compartments.

Resolution: Vessel submitted satisfactory class report attesting to the structural condition of the hull and watertight compartments.
THE PSCO OBSERVED DURING TESTING THE OWS 3-WAY VALVE THAT THE VALVE WAS UNABLE TO STOP ALL OF THE EFFLUENT ONCE THE VALVE WAS ACTIVATED. DURING OPERATIONAL TESTING THE 3-WAY VALVE WAS STILL ALLOWING EFFLUENT TO PASS BY WHEN IN THE CLOSED POSITION. REPAIR VALVE AND DEMONSTRATE PROPER OPERATION PRIOR TO THE VESSEL’S DEPARTURE FROM PORT.

Resolution: Received report from LR Class surveyor attesting to three-way valve being repaired, reinstalled, and tested satisfactorily. PSCO attended vessel and witnessed valve functioning properly.
DURING PSCO'S EXAMINATION OF THE OCM, IT WAS FOUND THAT THE SEAL WAS BROKEN, RENDERING THE RELIABILITY OF THE OCM'S PPM READINGS QUESTIONABLE. ADDITIONALLY, THE SEAL IS A CALIBRATION SEAL FROM THE MANUFACTURER THAT RENDERS THE CALIBRATION OF THE OCM VOID IF THE SEAL HAS BEEN BROKEN. PROVIDE OPERATIONAL DATA RECORDED BY THE OCM FOR THE PERIOD OF 01SEPT2016 TO 23SEPT2017 TO THE USCG FOR REVIEW PRIOR TO THE VESSEL’S DEPARTURE FROM PORT. ADDITIONALLY, A QUALIFIED TECHNICIAN IS REQUIRED TO CALIBRATE AND RESEAL THE OCM IN THE PRESENCE OF AN ATTENDING USCG PORT STATE CONTROL OFFICER PRIOR TO THE VESSEL’S DEPARTURE FROM PORT.

Resolution: On 26 Sep, PSCO witnessed technician from MacKay Marine replace the desiccant on the Deckma measuring cell and conduct a satisfactory operational test. On 28 Sep, PSCO witnessed tech install new measuring cell and conduct a satisfactory operational test.
THE VESSEL’S SAFETY MANAGEMENT SYSTEM REQUIRES THAT A “SECURITY SEAL SUPPLIED BY THE COMPANY SHOULD BE PUT ON THE OVERBOARD DISCHARGE PIPE. ONE BOLT OF EACH FLANGE ON THE OVERBOARD PIPE FROM OWS WILL BE FITTED WITH 3.0MM PRE-DRILLED BOLT HOLE AND LOCKED." THE FLANGES INTERCONNECTING 02 IN-LINE SHUT-OFF VALVES FOR THE OWS OVERBOARD DISCHARGE LINE LOCATED AT THE SIDE SHELL ARE CONNECTED WITH BOLTS THAT ARE NOT SECURITY SEALED. ABSENCE OF A SECURITY SEAL ON THE FLANGE BOLTS FOR THE OVERBOARD DISCHARGE VALVE ON THE OWS DISCHARGE LINE IS IN DIRECT VIOLATION OF THE VESSEL'S SMS, AND AS SUCH REPRESENTS A POTENTIAL MAJOR NON-CONFORMITY AS DEFINED BY THE ISM CODE. BASED ON THIS OBSERVATION, IT IS RECOMMENDED THAT AN EXTERNAL ISM AUDIT BE COMPLETED BY THE VESSEL'S SAFETY MGMT CERTIFICATE ISSUING AUTHORITY. PROVIDE NOTIFICATION TO USCG OF FLAG STATE DECISION ON WHETHER TO COMPEL AN EXTERNAL ISM AUDIT PRIOR TO THE VESSEL'S DEPARTURE FROM PORT. ADDITIONALLY, SECURE BOLTS TO OVERBOARD FLANGE IAW/VESSEL'S SMS PRIOR TO DEPARTURE FROM PORT.
