All access openings in bulk heads at ends of enclosed superstructors shall be fitted with doors of steel or other quivalent material, perminantely and strongly attached to the bulk head, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weather tight when closed. The access door to the provisions did not close as intended allowing exposure to the elements.

The machinery associated piping systems, and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board. The #4 cylinder of the main propulsion engine was leaking a significant amount of jacket (cooling) water.
