0720 Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. Upon witnessing crews testing of ship's rescue boat outboard engine, heavy white smoke was observed billwing from the engine, additionally PSCO observed that the engine was not dishcarging cooling water, as designed from reservoir of raw water supplied to engine's lower unit. PSCO observed engine sieze and cease operations. PSCO further observed the ship's crew attempt to start the engine again without success. It is the PSCO's assessment that the ship's rescue boat engine is not operational.74SOLAS(2014)III/20.2 30abc

0615 Cargo ships shall carry at lease one rescue boat complying with the requirements of section 5.1 of the code. Rescue boat equipment shall be secured within the rescue boat by lashing, lockers, compartments, or similar mounting arrangements. All required equipment for the rescue boat was absent from the rescue boat upon examination by the PSCO. The ship's crew provided the required equipment upon request, which was stored in the 3rd Officer's stateroom. PSCO also observed that the ship's crew had secured rags around the fuel injector housings for cylinders #'s 1, 3, and 5. 0663 An immersion suit of an appropriate size complying witht he requirements of section 2.3 of the code shall be provided for every person on board the ship. PSCO observed that the immersion suits on board were severely deficient in their material condition. Specifically the suit's watertight seams between the front zipper and the outer and inner layers of thermal protective insulation were delaminated. Vessel's crew failed to safely and effectively respond to a simulated fire scenario. During a PSCO initiated fire drill. PSCO provided ship's crew feedback on their performance and provided 30 mins to correct errors and conduct training. The ship's crew failed to safely and effectively respond to a simulated fire in 2 separate drill scenarios.
